NURSING ASSESSMENT

There are five stages of the nursing process are assessment, diagnosis, outcome
identification and planning, implementation, and evaluation. Assessment is the initial stage of the
nursing process and is a systematic process of collecting data from various data sources to evaluate
and identify the client's health status. There are two types of the assessment are initial/first
assessment and reassessment/ evaluation. There are five elements of the assessment process are
data collection, data verification, data organization, data interpretation, and data documentation.
There are two types of data are subjective data and objective data. Subjective data are
include chief complaint and patient history. Objective data are include physical examination and
diagnostic procedures. Physical examination or clinical examination is a process of a medical
expert examine the patient's body for signs of clinical disease. The example of physical
examination are general appearance, vital signs: T, BP, P, R, HEENT, lungs, back, heart, breasts,
abdomen, fundal height, estimated fetal weight, presentation extremities, vaginal examination,
and fetal monitor
To make objective and subjective data, data must be relevant to client needs, collected
from a variety of sources, collected using appropriate techniques, collected in a systematic
manner, documented in a usable format. As a nursing we must have strong communication skills,
consistence, up to date, assuring, effective, not judging.
How to communicate with patients are smile and make eye contact, call the patient by hi
or her last name, introduces yourself your name, role, and what you are going to do, why youre
qualified to do it, duration of task or test are length of time, process, procedure, waiting,
explanation whats next what tool you are using whos coming what you are doing and why, thanks
the patient like a stronger sense of involvement were glad you chose us. The benefit of that
communication are This conversational style allows you to clearly communicate, Getting
important pieces of information, Specifically gain trust, Increase compliance, Improve the clinical
experience.
If we dont know what to say to a patient, we can use a scripting, because scripted talk is
more than just an annoying quirk of the modern service economy. It represents a deep form of
managerial control-a regimentation of the labor process so total that it extends even to speech.
Scripting make a nurse increasing the communication to patient so patient increasing satisfaction
scores. Nurse or Midwife has to ensure that their message is clear and consistent and to avoid
potential misunderstanding. Consider scripting if you think you will have a problem in
communication
