SENIOR QUESTIONNAIRE
NAME:__Carrie Lewton___________________________________________________
DATE:__________________________

To assist the recommender in representing you as accurately as possible in the letter,

please read and respond to the statements and questions below. The care and thought you
give in addressing these prompts may reiterate what the recommender already knows about
you or may help them discover something previously unobserved. In any event, take nothing
for granted.

1. Explain the qualities that you feel you possess which might easily go unnoticed by
others.
I consider myself to be a very empathetic and caring person. I often find myself doing
things that may not benefit me, but will benefit others significantly, without expecting
anything in return. I also make it a priority to check on the people who I value every
now and then, as you are often unaware when people are struggling. Being a naturally
shy and introverted person I find these gestures often go unnoticed by a majority of
the people around me.
2. Complete the following statement as clearly and directly as you can: I am especially
proud of the following achievement.
I am proud of the work I put in to improve my mental health throughout my high
school career. I began high school in a very bad place and have been determined to
change that for the duration of my high school career. I have done anything I could to
improve this aspect of my life, and I am truly proud of that.
3. Mention any factors that have come up during your high school career which you feel
have put you at a disadvantage.
As previously mentioned, my freshman and sophomore year I really struggled with
mental illness, however I have been working incredibly hard since then to improve
that aspect of my life. It has been a struggle, but Im really proud of how much I have
improved and the coping skills I have learned.
4. Do you feel you present grades are a fair representation of your academic ability? If
not, discuss.
I do not believe my grades are a fair representation of my academic ability. Early on in
high school I was forced to put a majority of my focus into my mental health. I feel if I
had the ability to go through high school in the mindset I have now, I would be able
to have much better grades and overall bo more focused on academics.
5. What goals have you set for yourself and what are you presently doing to achieve
them?
My personal goal has always been to become a therapist. To achieve that I have taken
every psychology class that is offered at Athens as well as an independent study in
psychology.
6. What activities have you pursued and to what extend? Mention any offices you have
held and awards won. Tell what you gained from this experience.
In the beginning of my high school career I did choir and theatre, however I quickly
realized those activities were not for me and did not participate again my following
years. Now I have chosen to focus on volunteer work, including that in environmental
club.
7. In what community activities have you participated?
I figure skated for a majority of my childhood, and recently participated in a program
that allowed underprivileged children from Detroit to receive skating lessons. I gave
lessons to the younger children as well as help them learn to take skates on and off
as well as the ice rules.
8. Explain your personal contribution to the high school community.
I transferred to Athens the second semester of my sophomore year, and since then IK
have made it a priority to attend all the school events I possibly can. This includes
dances and other school funded activities. I personally believe these most of these
activities are fun ways to provide additional funding to certain programs.
9. What are your hobbies?
I personally enjoy painting and drawing. While I may not be the best at either, I find
both to be very calming and therapeutic in nature. In addition, I enjoy to cook for
myself and my family. I really like to try new recipes I find online or even create my
own.
10.Describe any independent study or special programs/projects in which you have
engaged (science, drama, music serious extra work in a subject.)
In my sophomore year I did an independent study in psychology, specifically the topic
of social psychology. I found this to be incredibly informative and insightful, as I
learned a lot about the way people behave and respond to others.
11.Describe any work experience you have had - paid or unpaid. Describe your
responsibilities, dates of employment, average hours per week, etc.
While I have not officially been employed, I have babysat for my neighbors and some
of my mothers co-workers. I was paid for this service, however it was very informal
and I did not receive an official paycheck. Therefore, I do not have official dates of
employment.
12.Describe your summer experiences and what you gained from them:
a. After freshman year: This summer is when I began to focus on bettering my
mental health. It was a journey from there, however I had a lot of courage to
admit there was a problem.
b. After sophomore year: This summer allowed me to experience what it was like
to feel alive. I found I was doing better mentally than I had been in years and
was able to really just enjoy life as it is.
c. After junior year: hasnt happened yet will keep you updated

13.Who are two Athens teachers to whom your counselor might go for further reference?
I attended Troy High for my freshman and a majority of my sophomore year, so the
first teacher I would go to would be my chemistry teacher there, Ms. Barrett. Then I
would also go to Ms. Tucci here, she helped with my independent study and is my AP
psychology teacher this year.
14.Where will you apply/have you applied to college?
My first choice where I plan to apply is Wayne State. After careful consideration of the
pros and cons of nearby universities, I found this one had the greatest amount of pros
and the least amount of cons. The most important factor to me, being the well known
psychology program the school has.
15.What is your intended major?
I intend to work towards receiving a bachelors degree in psychology.
16.How did you become interested in this major?
I have always found psychology to be an incredibly interesting subject, however my
own personal struggle with mental illness solidified my choice of choosing it as a
major.
17.What are your intended career plans?
I plan to become a psychological counselor or therapist. I really would like to do
something that allows me to make it easier for teenagers struggling with mental
illness in the way that I have in the past.
