1. What goal did you meet this week that you and your preceptor discussed? What

This week my preceptor discussed with me that I needed to get all of the practice that I can with
doing full assessments on every patient. She suggested that I read in the Uphold and Graham
book each night about different disease processes to gain a better understanding of what types of
focused questions to ask with each specific type of patient. For example diagnosis of diabetes,
hypertension, and joint pain, which were some of the types of patients seen during todays

The first patient I encountered was being seen for a follow up visit for diabetes. Her last

recent diet, to find that she has been under stress lately and had been consuming salt and vinegar
potato chips each night. I consulted with my preceptor about current diabetes medication
regimen and asked what appropriate medication changes would be for this patient. I spoke with
the patient about the importance of diet in diabetes management and offered suggestions of only
consuming portion size as well as different types of snacks that would be healthier options for a

presented with a chief complaint of lower back and right hip pain, which had been ongoing for

learning about available resources that will benefit these patients. For example, this week I
