PETITION

Come now the petitioners, through the undersigned counsel,

unto this Honorable Court respectfully alleges that:

1. Petitioners are husband and wife, of age, Filipino citizens

and residents of Brgy. M.V. Hechanova, Jaro, Iloilo City, machine
copy of their Certificate of Marriage is hereto attached as Annex
A;

2. Petitioners are childless, without any adopted child and

intend to jointly adopt the minor ISABEL ALAIKA CO, nine (9) months
of age, who was born on October 02, 2013 at Iloilo City;

3. The minor Isabel Alaika Co is the illegitimate child of Vicky

M. Belo-Co, younger sister of Petitioner Emme Mendoza, and is
presently residing at Brgy. M.V. Hechanova, Jaro, Iloilo City. A copy
of the Birth Certificate of the adoptee is hereto attached as ANNEX
B;

4. The minor Isabel Alaika Co is not disqualified by law to be

adopted and has no estate or property;

5. Petitioners are in possession of full civil capacity and legal

rights; of good moral character and have not been convicted of any
crime involving moral turpitude; emotionally and psychologically
capable of caring for children; are at least sixteen (16) years older
than the adoptee; and are in the position to support and care for their
children in keeping with the means of the family and have
undergone/will undergo pre-adoption services as required by Section
4 of Republic Act No. 8552. Machine copies of their Certificates of

Live Birth are hereto attached as Annexes C and D,

respectively;

6. The biological mother of the adoptee, Vicky Belo-Co has

given her consent to the adoption of her daughter, Isabel Alaika Co,
as shown by her Affidavit of Consent, copy of which is hereto
attached as ANNEX E;

7. Also attached as Annex F is a copy of the Baptismal

Certificate showing the name, date and place of birth of the adoptee.

PRAYE R

WHEREFORE, it is most respectfully prayed that upon due

notice, publication and hearing, a decree of adoption be issued in
favor of petitioners to the effect that henceforth, the child ISABEL
ALAIKA CO be freed from all legal obligations of obedience and
maintenance with respect to her natural parent, and be, to all legal
intents and purposes, the child of Petitioners, and that the surname of
said child be changed to MENDOZA, which is the surname of the
Petitioner Spouses;

pending with the Supreme Court, the Court of Appeals or any other
