Instructions are included for caring for. maintaining, clearing, disinfecting, and using the Prima imaging system. For the purposes of this manual, the use of the term Prime will refer to the SONOLINE Prima imaging system. Conventions used throughout this manual are fisted below. Introducing the SONOLINE Prima Diagnostic Ultrasound Imaging System ‘The SONOLINE ® Prima diagnostic ultrasound imaging system provides complete clinical images in a variety of real-time imaging modes. The Prima imaging system allows you to perform a wide range of diagnostic ultrasound ‘examinations, including those for obstetrics, gynecology, and urology, with a standard calculations and measurements software package provided. The Prima c imaging system can be configured to support your diagnostic requirements. ‘The Prima imaging system is compact and self-contained, and can be ‘maneuvered into exam sites where floor space is limited. The imaging system ‘can navigate doorways, elevators, mobile van doors, and small examination rooms such as those in clinics, in doctor's offices, and in emergency rooms. The imaging system can also be transported long distances with only a limited amount of disassembly. The Prima imaging system is compatible with a variety of linear and curved array and mechanical sector transducers. The configuration of an individual imaging system defines the transducer types available on that particular system. The layout of the control panel facilitates quick recognition of control locations. The keyboard controls and keys: wd) are backlit, and receive additional task lighting for easy identification in areas with dim lighting. The backlighting increases on a control key when itis pressed, to indicate that the function is active, On the 118V version of the Prima imaging system, each control on the control panel is labeled with an icon and English text. On the 230V version, the controls are labeled with icons only. Both versions of the system support menus and reports in English, German, French, and Spanish, ‘The Prima imaging system supports a variety of optional documentation devices such as a video printer. The Prima imaging system can also accommodate optional documentation and storage devices, including: * Video cassette recorder (internal or external mount) * Multisformat camera (external) * Report printers (external) ‘The documentation and storage devices that are connected externally operate as separate units. These devices are not directly controlled from the Prima imaging system. The monitor screen can also display graphic overlays, and features such as calipers and pictograms, to assist in evaluating the image. The information that is displayed on the right side of the imaging ‘screen can change as a result of your current activity. This area normally displays the System Status Menu, which provides information about the current imaging mode, unless you: Initiate a function that displays another menu, such as the Measurements Menu, in place of the System Status Menu. Certain imaging parameters are stil displayed at the bottom left of the screen, unless General System Set Up Menu 1 has been used to tur off the System Status Parameters display. = Press F1 New Patient, F2 Report, or F3 Patient Data to display a page of patient information or press F4 Preset Menu to display a General System Set Up menu. The different imaging menus are described on page 1-15. The General System Set Up menus are discussed in detail in Chapter 6. When image information is being played back from a video input source, such 185.8 VCR, the screan saver function is turned off. See Chapter 8 for instructions on Using a VCR with the system. The image display screen is covered by a screen saver graphic after a specified period of time. You can use General System Set Up Menu 1 to determine the length of time before the screen saver is displayed. Refer to Chapter 6 for specific information, ‘The screen saver is turned off and the previous display is restored when you ress a key of adjust any of the controls. Only one of these menus is active at a time. Imaging parameter data is always displayed at the bottom left of the imaging screen, unless it has been turned off using General System Sat Up Menu 1. System Status Menu The System Status menu indicates the status and parameters of the current imaging mode as well as frequency, depth, and transmit power level of the active transducer. Refer to Chapter 5 for information on imaging modes, and Chapter 6 for information on exam-specific transducer settings. Measurements Menu The Measurement menu is displayed in the upper right corner of the screen when the CALIPER key on the control panel is pressed. Exam-specific parameters are displayed in the upper section of this menu. Measurement methods are listed and calculation results are displayed in the lower section. Refer to Chapter 7 for detailed information on Calculations and Measurements. Disk Menu ‘The Disk Menu is displayed in the upper right comer of the screen when the DISK key on the control panel is pressed. This menu provides options for storing and retrieving images using the floppy disk drive. Refer to Chapter 5 for specific information. B-mode 2B-mode B+B-mode 4B-mode C Bamode B+B/M-mode A-mode & B-mode is the default setting. When you power-on the system, the imaging screen is initially displayed in B-mode. A single active B-mode image is displayed in the image area. 2B-mode displays two separately acquired B-mode images, side by side. 2B-mode allows you to switch from one B-mode image to the next. B+B-mode displays two identical B-mode images, side by side. ‘The two images are simultaneously either real-time or frozen. Each image can show different post- processing functions. B+B- mode can also be used with ECG-triggered image acquisition. 4B-mode displays up to four separately acquired B-mode images at the same time. 4B-mode is useful for comparing a number of ‘anatomical views on one screen or film. Only one image at a time can be displayed in realtime. M-mode displays a full-screen M-mode sweep. M-mode is also called Time Motion Mode. B/M-mode simultaneously displays a B-mode image with an M-mode sweep. Several display formats are available. B+B/M-mode simultaneously displays a B+B-mode image with an M-mode sweep. ‘Acmode displays an amplitude readout in graph form on the left side of the active B-mode or M-mode image. The ultrasound output power levels are stated to permit the user to critically evaluate the system settings in the event of new research findings being announced. Refer to Appendix B for information concerning acoustic output intensities. The effects of different imaging functions on acoustic output are explained on page 1-26. Power Control ‘Acoustic power is the amount of energy applied directly to the patient. The POWER control on the control panel adjusts the acoustic output power delivered from the transducer to the patient. The POWER control determines the ‘transmitted level of ultrasound for all transducers and imaging modes during reat ‘time imaging, The range and especially the maximum level of acoustic power will differ depending on the transducer. In addition, each diagnostic exam type has preset ower values. Extreme care should be used when increasing the transmit power above the preset limits. To adjust the transmit power: = To increase the transmit power, rotate the POWER control clockwise. a When the maximum power level is reached, the imaging system - + emits a beep. "To decrease the transmit power, rotate the POWER control counter- clockwise. When the minimum power level is reached, the imaging Powor system emits a beep. The transmit power ranges from a minimum of -24 dB to a maximum of 0 dB in 3-dB increments. Specific information is provided in the following table. ‘Operation ‘Effect on tha acoustic output Transducer change “The maximum acoustic output of each transducer is optimized to produce the best image quality within FDA guidelines. Thus, the acoustic output will change as the operator changes the active transducer. Imaging mode change _| Since B- and M-modes use different default imaging parameters, changing the ‘mode will change the acoustic output power of the system, No changes occur when switching from B to 2B or 4B, since the basic imaging parameters remain the same, In most cases, the acoustic output powel for M-mode is larger than in B-mode, however, it depends on the specific oresets for B and M-mode, Field of View (Sector Angle or Scan Width) ‘Changing the sector angle or scan width may result in changes to the frame fate, and thus change the acoustic output. Image depth change ‘Changing the image depth changes the PAF, and thus changes the acoustic output. Number of focal zones Since the number of focal zones influences the frame rate and the actual position of the focal zones, changing the number of focal zones changes the ‘acoustic output. Focus location ‘The transmit focus location change will couse the acoustic output change, ‘even though the transmitting electrical energy level and the aperture remains the same. In most cases, the acoustic power will increase ithe focal point is moved closer to the transducer. Zoom Activating zoom may change the focal depth and aperture. The worst case ‘acoustic output condition is from the largest transmit aperture at a given depth. Thus, acoustic output changes depending on the zoom locaton. Freeze ‘Activating the freeze {unction stops the electrical energy transmit part of the system, thus disabling the system from generating ultrasound. ‘Automatic time-out When the Prima is inactive for a selectable time period, the system automatically freezes and displays the screen saver. Activatng the freeze function stops the electrical energy transmit part of the sysiem, thus disabling the system from generating ultrasound, The timeout can be specified using the General System Set Up menus. Transmit power ‘The transmit power level change wil change the electrical output of the system to the transducer, and thus change the acoustic output. Frequency change Changing the operating frequency changes the focal characteristics of the acoustic waves, thus changes the acoustic output. Electrical safety tests must also be performed at regular intervals in accordance with safety regulations, or as required. Siemens Authorized Care Installers and operators must observe all regulations that govern the installation, operation, inspection, and maintenance of this equipment. To ensure the safety of patients, operators, and third parties, the equipment must be inspected every 12 months, and replacement of parts performed as necessary. This maintenance must be performed by a qualified Siemens service representative. It is important to inspect the equipment more frequently if it is operated under extraordinary conditions, Is important to perform inspection and maintenance at the prescribed intervals to avoid allowing any part of the system to become hazardous due to wear. Use Universal precautions when cleaning and disinfecting. You should treat all portions of the imaging system that come in contact with human blood or other body fluids as if they were known to be infectious. All exterior parts of the system, including the control panel, transducers, and biopsy devices, should be cleaned andior disinfected as necessary or between uses. Clean each component to remove any surface particles. Disinfect the components to kill vegetative organisms and viruses. Cleaning and Disinfecting the Prima Ima: ig System WARNING: To avoid the possibilty of electric shock and damage to the system, turn off and disconnect the equipment from the power receptacle before cleaning and disinfecting WARNING: To avoid health hazards, comply with the disinfectant manufecturer's instructions. CAUTION: To avoid the possibilty of static shock and damage to the system, avoid the use of aorasol spray cleaners on the monitor screens. CAUTION: Do not clean the system with chlorinated or aromatic solvents, acidic ‘or besic solutions, or strong cleaners such as ammoniated products, as the: damage the surface of the system. Use the clesning procedures described beginning on page 1:38. CAUTION: Do not use spray cleaners on the system, or pour fluid onto the system surfaces, as fluid can seep into the system and damage electronic components. It also possible for the solvent fumes to build up and form figmmable gases or damage intemal components. Use a clean gauze pad or lint-free cloth, lightly moistened with a mild detergent, 10 wipe the surface of the system. ‘Take particular care to clean the areas near the trackball and the DGC slide controls. Ensure these areas are free of gel and any other visible residue. Be sure cleaning solution does not seep into the control panel keys, keyboard, or any other openings. 3. Alter cleaning and disinfecting, reconnect the system to the power receptacle. Occasional int and gel may collect around the trackball To clean the trackball: 1. Use a soft, dry brush, like a toothbrush, to remove dried particles from the area around the trackball. 2. Wipe the entire surface of the trackball with a lint-free gauze pad, lightly moistened with a mild solution of water and detergent. The backlighting intensity increases on a key when itis selected. This allows you to see, at a glance, which function is active and which keys and controls are currently in use. The alphanumeric portion of the control panel is not backlit. The switch on the underside of the monitor controls whether the control panel light is on or off. There are two methods for adjusting the level of lighting; once a level is selected, the system will provide that level of lighting whenever the switch is turned on. POWER ‘The POWER (Transmit/Output Power) control adjusts the overall transmitted power delivered through the transducer to the patient. The POWER control operates in all ultrasound imaging modes and with all types of transducers. Default power values are dependent on the exam that is currently selected. B/M-GAIN ‘The B/M GAIN control adjusts the overall system receiver gain for B-mode and M-mode. The B/M GAIN control can be used to adjust the return echo amplification and brightness of the B-mode image or M-mode sweep. Procedures ‘are included for performing the Daily Checklist, installing the power cord and powering ‘on the system. In addition, this chapter provides a general system software installation ‘procedure and describes how to enter general system settings. System Setup ‘The Prima imaging system is a self-contained unit that can be moved or transported easily. Each day before you use the Prima imaging system, perform the Daily Checklist procedures described below. Daily Checklist Perform the following checklist items each day before you use the system: Q Visually inspect all transducers. Discontinue using a transducer that has a cracked, punctured, or discolored transducer face, or @ frayed cable. G_ Visually inspect the power cords. Do not tum on the power if the power cord is frayed or split, or shows signs of we: Disconnect the power cord from the wall outlet, observing the proper precautions. Q Verify that the control panel, including the trackball and DGC controls, appears clean and free from gel or other contaminants, (Once the system is powered on: Verify that the monitor displays the current date and time. System Set Up System Startup Starting up the Prima imaging system requires you to install the power cord and Power on the system. Installing the Power Cord WARNING: For 115V systems, in order to ensure grounding reliability, connect the system only to a hospital-grade receptacle. To install the power cord: 1. Locate the power cord. 2. Connect the female connector at one end of the power cord to the power connector on the rear panel of the system. 3. Perform the Daily Checkiist described on page 33. Verity that the power cord is plugged in. 3. Make sure that the circuit breaker on the back of the system is in the ON position, 4. Push the power switch into the ON position. The power switch is located at the upper left side of the control panel When the system is first powered on, it runs through a series of self- diagnostic tests, which last for approximately 30 seconds. The system is then ready to use and the imaging screen is displayed. The system is factory-configured to default to B-mode. To power the system off: Press the power switch to the OFF position. Wait at least five seconds before you power the system on again, to allow all of the components to complete their shutdown sequence. The circuit breaker on the back of the system should remain in the ON position L sno even SONOLINE PRIMA OPERATING INSTRUCTIONS Min 3-7 CHAPTER a System Set Up Moving the System ‘The Prima ultrasound imaging system is designed to provide a high level of ‘mobility and is easily transported from site to site. Before moving the system to another location, you must power the system down and secure all accessories and moving parts. To secure the system for transportation: 1, Press the power switch to the OFF position. 2, Unplug the power cord from the power source and store the power cable safely. 3. Monitor. The monitor can be removed from the Prima system for transportation in a car or van. For information on removing and attaching ‘the monitor, see page 3-11 ‘The monitor can be secured for travel using the transport locking levers located on the right side of the monitor base. If you carefully swivel the ‘monitor until it faces forward, and then tit it into ¢ fully horizontal position, the tit-and-swivel levers will lock into position with an audible click. Transducers can be connected and disconnected while the Prima system power is on or off. If the system is on, press the FREEZE key to freeze the image before adding or removing a transducer and again after the change is complete, 10 enable the system to recognize the new transducer. ‘Transducer Holders and Transducer Cable Holders ‘The transducer holders on the sides of the control panel are interchangeable and replaceable. This secures the connector in position and ensures the best Possible contact, ‘Mechanical sector 3. Place the transducer in the transducer holder and drape the cable transducer port through the cable holder. For endocavity transducers, you may want to place the transducer in the horizontal tray located at the back of the system below the monitor. To disconnect a mechanical sector transducer: CAUTION: To avoid damaging the transducer cable, do not pull on the cable to disconnect the transducer. Use the following instructions. 1. System Set Up Connection of Peripheral Equipment oD In order to fuffil EC 601-1-1 (Medical Electrical Equipment, Part 1: General Requirements for Safety) requirements, connection of peripheral equipment to your ultrasound imaging system must adhere to one of the following conditions: ‘+ The peripheral equipment itself is a medical device approved according to IEC 601-1, or + Non-medical peripheral equipment approved according to any other IEC standard (IEC xxx) must use the followirg setup for Connection (refer to the illustration on the following pagel: Connect the imaging system to an independent protective earth terminal, with a ground wire ‘connection to the imaging system's Equipotential Connector. Ensure that the protective earth wire is eee connected to a qualified protective earth connection eau independent of the existing system’s earth connection system. In B+B-mode, the focal zone(s) must be the same for both images. In 2B-mode and 4B-mode, each image can have a different number of focal zones. Focus and Frame Rate Maximum frame rates are achieved when a single focal zone is used. Frame rates are reduced when two focal zones are selected, and reduced stil further when four zones are used. The amount of frame rate reduction depends on the transducer and the depth of view. Positioning Focal Zones ‘A single focal zone can be positioned anywhere in the field of view, including in the first or last centimeter of the depth scale. The distance between two focal zones is fixed, and determined by the field of view. pair of focal zones can be repositioned if the field of view is deep enough. Detaled information is provided on using he Gener! System Set Up menus and submenus to configure the system for general operation, specific ‘exam types, nd powern. An explanation i provided foreach manu selection. Overview ‘The Prima imaging system software can be used to configure the system to support your diagnostic requirements. Menus are provided in which you can specify pre-defined settings, or presets, for your imaging preferences. For example, you can select default settings for your most frequently used exams and transducers. Your imaging preferences and presets can be stored on diskette and loaded into the system as needed. When you exit the menus, the system retains the selections and definitions you have defined. It is advisable to back up your system presets on a diskette, to prevent accidental loss of your information. Presets saved on a diskette also expedite ‘the installation of a new system software release. See Chapter 8 for details on saving your presets to a diskette. To display the General System Set Up menus: 1. Press the F4 Preset function key on the keyboard, ‘The system displays General System Set Up Menu 1 on the image screen. 2. Press the NEXT key on the control panel to cycle through the menus from 1 to 5. If you know which menu you want to view, press the corresponding number on the keyboard. NOTE: Instructions for the available actions are displayed at the bottom of ‘each Sot Up menu, To change a menu setting: wed) 1.
