1. OBJECTIVE

This instruction identifies the procedure to be followed for the issue of


Safety Documents.

The following Safety Documents will be used to cover:-

i) Access - this document is used to specify the limits of work and/or


access when this is sufficient alone to achieve Safety from the
System.

ii) Permit - this document is used when precautions need to be taken to


achieve Safety from the System for work on Plant and/or
Apparatus.

2. SCOPE

The procedure is intended to be used by all parties involved in the


commissioning and initial operation of the power station equipment. It is
important that all persons associated with these activities are familiar with
this Management Instruction and its correct implementation.

3. DEFINITIONS

Terms printed in bold type are as defined in the Safety Rules.

4. PROCEDURE

4.1 Plant/Apparatus Release from Service

In order to manage commissioning and operational activities,


requests for Safety Documents must be made in advance. Where
Plant and/or Apparatus has to be released from service this shall
only be done with the approval of Site Manager's Nominee.

The person requesting the Safety Document shall give details of:
i) Work commencement date and time
ii) Plant and/or Apparatus to be worked on

iii) Adequate description of the work in order that adequate


terminal isolation and/or earthing, "Safety from the System"
precautions can be effected by the Safety Controller.
iv) Anticipated duration of the work.

Plant/Apparatus released from service will be made safe for work in


accordance with the appropriate Safety Rules and supporting Safety
Codes of Practice, Management Instructions and Local Safety
Instructions.

4.2 Issue of Safety Document

Having confirmed that precautions have been taken to achieve


Safety from the System, the Safety Controller can prepare the
Safety Document.

The Safety Controller shall enter the following details as


appropriate:

i) Identification of Plant and Apparatus

ii) Work to be done

iii) Precautions taken to achieve Safety from the System

iv) Further precautions to be taken during the course of work to


avoid system derived hazards.

v) Safety precautions which may be varied by the Acceptor

vi) A list of items to be issued with the document as appropriate:


- portable Drain Earths and Earthing Schedule
- procedure for Restoration of Supplies
- specialist advice on particular hazards

vii) Whether the Safety Document limits work to a Single


Working Party document.

viii) Signature, time and date

The keys used to lock off Plant and/or Apparatus shall be safely
secured in a key safe which will be held in the Designated Permit
Office(s). The key securing the key safe will be attached to the Safety
Document and held by the Acceptor providing Supervision for the
duration of the work.

Fuses links, vents plugs, etc. which have been removed by the
Designated Person to achieve Safety from the System shall be
safely secured in the Designated Permit Office(s) until the Safety
Document is cancelled.

A Safety Document must be issued to an Acceptor by an Issuer.

The Issuer shall ensure that the listed items to be issued with the
Document are made available to the Acceptor.

The Issuer shall ensure that the Acceptor reads the Safety
Document and understands the limits of the work, can properly
identify the Plant and/or Apparatus and is conversant with any
further precautions or special procedures to be adopted.

It is important to discuss any additional precautions or special


procedures the Acceptor must adopt prior to commencement of the
work. If there is any query, the Acceptor should clarify this with the
Issuer issuing the Safety Document.

After signing the Acceptance section of the Safety Document the


Acceptor retains the hard copy together with the key safe key. The
flimsy copy shall be retained in the Designated Permit Office(s) for
record purposes.

4.4 Surrender and Transfer of Safety Documents

During the course of work it may be necessary to surrender the


Safety Document for operational reasons and then reissue it later
for work to restart. In all cases in order to ensure that the transfer of
responsibility

of Supervision is clearly enacted and recorded, the Safety


Document transfer system must be followed.

Safety Documents shall be transferred between Acceptor by


Issuer. Any such transfer shall be to the same standard as the
original issue after the original Acceptor has surrendered the Safety
Document to an Issuer.

4.5 Work Completion and Safety Document Clearance

When the work is completed, the Acceptor responsible for the work
at the time of work completion must ensure that:

i) all persons in his Working Party have been withdrawn from


and warned not to restart to work on the Plant/Apparatus
concerned.

ii) all gear, tools and loose materials have been removed and
guards and access doors replaced, apart from any exceptions
identified by the Acceptor.

The Acceptor can then sign the Clearance section of the Safety
Document and return the document and key safe key to the
Designated Permit Office(s).

4.6 Cancellation of Safety Documents and De-isolation

The Safety Controller shall cancel the Safety Document by


signing the Cancellation section of the document ensuring that
appropriate actions are taken in respect of any exceptions quoted by
the Acceptor in the Clearance Section(s).

After the cancellation section has been signed the Safety Document
will no longer be valid and the Plant and/or Apparatus can be de-
isolated and made available for service.
