JOINT AFFIDAVIT
OF TWO DISINTERESTED PERSONS
We, PATRICIO L. BUMATAY, of legal age, Filipino citizen and a
resident of _______________, and BENJAMIN S. MANANGAN, of
legal age, Filipino citizen and a resident of _______________, after
having been duly sworn to in accordance with law, do hereby depose
and state THAT:

1. We both personally know ARTEMIO DUQUE ALETIN

(Artemio, for brevity), who was born on June 18, 1939 to
Spouses Esiminiano Aletin and Tomasa Duque in __________;

2. Artemio is a member of the Social Security System of

the Philippines (SSS) until his death on March 21, 2017.

3. When _______, the ____ of Artemio, went to the SSS

Camiling, Tarlac to file for the ___________ of ___ late
_______, ___ was required to submit/present, among others,
copies of the Birth, Marriage and Death records of Artemio;

4. Upon submission of the required documents, it was

discovered that the recorded Name of Artemios father appearing
in his Marriage Contract is PERFECTO ALETIN, instead of
ESIMINIANO ALETIN as recorded in his Certificates of Birth and
Death. Hence, the discrepancy;

5. We are not related to Esiminiano either by

consanguinity or affinity, except that of being his __________;

6. We are executing this affidavit to attest to the fact that

the names ESIMINIANO ALETIN, as the father of Artemio Aletin
or Artemio Duque Aletin, appearing in his Certificates of Birth
and Death, and PERFECTO ALETIN, as the father of Artemio
Aletin, as recorded in his Marriage Contract, refer to one and the
same persons, and that is the father of our barangay mate, now
deceased;

7. Finally, we are executing this Affidavit to attest to the

truth of the foregoing statements and for all legal intents and
purposes it may serve.
 IN WITNESS WHEREOF, we have hereunto affixed our
signatures this 5th day of May, 2017 at Camiling, Tarlac, Philippines.
