JOINT AFFIDAVIT
OF TWO DISINTERESTED PERSONS

We, MANUEL JR. I. MARACHA, of legal age, Filipino citizen

and a resident of Barangay Sinilian Cacalibosuan, Camiling, Tarlac
and EUGENIO L. CALIBOSO, of legal age, Filipino citizen and a
Barangay Sinilian 1st, Camiling, Tarlac, under oath, do hereby
depose and state THAT:

1. We both personally know MARIA CAROLYN DIMAYA TABILE

(Maria Carolyn, for brevity), born on December 24, 1984 to
Spouses Luisito Tabile and Jessie Dimaya in Espiritu, Ilocos
Norte;

2. Maria Carolyn is also known as MRS. MARIA CAROLYN

TABILE CORDERO after she got married to Robert M.
Cordero;

3. Maria Carolyn had been using MA. CAROLYN as her first

name since she came to the age of reason; hence, this is
what is reflected in her pertinent documents including her
school records except in her Certificate of Marriage;

4. When Maria Carolyn secured a copy of her Certificate of Live

Birth from the National Statistics Office (now Philippine
Statistics Authority), she discovered that her name
appearing therein is MARIA CAROLYN D. TABILE; Hence the
discrepancy in the first name of Maria Carolyn appearing in
her pertinent documents and school records;

5. We are not related to Maria Carolyn, either by consanguinity

or affinity, except that of being her barangay mates;

6. We are executing this affidavit to attest to the fact that the

names MARIA CAROLYN D. TABILE as appearing in the said
Certificates of Live Birth and Marriage, MA. CAROLYN
DIMAYA TABILE, as recorded in her pertinent documents
and school records, and MRS. MARIA CAROLYN TABILE
CORDERO, refers to one and the same person, and that is
our barangay mate;
 7. Finally, we are executing this Affidavit to attest to the truth
of the foregoing statements and for all legal intents and
purposes it may serve.
