Reflection on Artifact

AVIATION SAFETY AND HUMAN FACTORS - Flight Operations

Relates safety practices in diverse contexts, and applies to future learning and experiences.
For my artifact I chose that covers this learning criteria is a presentation that I created for
an accident report. I looking at the aircraft accident of flight 1455 and analyzed the ntsb report on
it and also the recommendations that came out of this accident so that something like it would
not happen again.
Analyzes system processes and environment to identify potential hazards or predict future
problems. Shares information with the intention of creating change within the
organizations safety culture and practices.
This artifact that I have here is a great example of a safety issue and then also a
recommendation to make that safety issue safer. As a pilot in this industry you have to be able to
make changes and improvements especially in safety. As a company you also have to be able to
hire qualified pilots and you need to know that even though a pilot is qualified they can still have
accidents. Just as shown in my artifact.
Pursues continued training and education on human factors related issues. Proposes input
on procedures that support safety-related operations.
This artifact shows that the cause of the accident was human error. This is why we need
to be on top of our game when we fly every single time. Pilots also have whats called iamsafe
checklists to help prevent accidents like this one here in my artifact. Where if the pilot is not in
proper condition to fly they dont fly.
