Lesson Plan Template

PLANNING
List the Common Core/State Standard(s) to be addressed in this lesson:

Analyze patterns and relationships.

3. Generate two numerical patterns using two given rules. Identify
apparent relationships between corresponding terms. Form ordered
pairs consisting of corresponding terms from the two patterns, and
graph the ordered pairs on a coordinate plane. For example, given the
rule Add 3 and the starting number 0, and given the rule Add 6 and the
starting number 0, generate terms in the resulting sequences, and observe
that the terms in one sequence are twice the corresponding terms in the
other sequence. Explain informally why this is so.

3. Research and information fluency

Students apply digital tools to gather, evaluate,
and use information.
d. Process data and report results

List the Learning Objective(s) to be addressed in this lesson. Use the

following format: Students will be able to

Students will be able to generate a pattern using two rules and find the
pattern or relationship between the terms, they then will be able to graph
them on a coordinate plane.

Describe how the objective is relevant to students lives.

Life has a lot of cause and effect situations so this math concept can help
show students a way that cause and effect really works.

List the words relevant to the content area that you will either introduce
and/or review during your lesson.
Input, output, function table, coordinate and coordinate plane.

List the materials you will need to teach the lesson.

White board/smart board, white board markers, and laptops.

ASSESSMENT
Pre-Assessment: Describe the instrument or process you will use to
measure students level of understanding toward the learning objective(s)
prior to teaching the lesson.

APPENDIX: Include a blank copy of the lesson pre-assessment. (if

applicable)

Is your pre-assessment included at the end of this lesson plan?

Describe the timeline as to when you plan to administer the pre-

assessment?
(Recommended timeline is a minimum of two days prior to teaching your
lesson).

Create and insert a table/chart/graph that shows the pre-assessment data

results. (if applicable)
Insert an image of your table/chart/graph here.
Describe how the results of the pre-assessment (what the students have
demonstrated they know) will be used to design the lesson objectives,
instruction, and post-assessment.

Post-Assessment: APPENDIX: Include a blank copy of the lesson post-assessment

you will use to measure students level of understanding toward the learning objectives after
teaching the lesson.

Is your post-assessment included at the end of this lesson plan?

APPENDIX: Include a copy of a Key/Product (completed by you, the teacher) which

provides a model of the desired outcome.

Is your key included at the end of this lesson plan?

TECHNOLOGY
Describe the instructional and/or assistive technology that you plan to
incorporate into the lesson to enhance instruction and student learning.

I will use the Smartboard during my direct instruction time. I will

demonstrate examples on the smart board. I will also pull up a smartboard
activity based on the lesson for students to be engaged and to help them
practice. I will also have the students create a coordinate plane on their
laptops. They will use the website
https://scratch.mit.edu/projects/37908984/#player to graph their data from
the function chart they created.
ACCOMMODATIONS
Describe the accommodations/differentiation/modifications you will use to
meet the needs of all learners and accommodate differences in students
learning, culture, language, etc. * Be sure that these accommodations are
based on what you identified/described in your contextual information
(Task I).

I will follow all IEPs.

MANAGEMENT
Identify the management and motivational strategies you will use to meet student
behavioral/developmental needs in order to keep students on task and actively engaged
throughout the lesson.

I will have create a poster during my lesson that the students can look
back at with a function chart example and the equation used. I will keep
class at a brisk pace. I will walk around my classroom and ask engaging
questions to keep students on task.

LESSON IMPLEMENTATION
I Do
(Teacher introduces lesson and models expected outcome of
learning objectives)
Describe how you will activate student interest and present the learning
objective in an engaging way (this is your lesson opening).
I will start my class by using a call back to get the attention of the
students. I will then ask them to get out their notebooks for math with their
laptops underneath their desks. I will remind them that we have been
talking about number patterns and today we are going to continue learning
about them by using a function chart.

Describe how you will communicate (to students) how the objective is
relevant to their lives.
I will talk about cause and effect and how that often times we do
something in life (the cause) and we will then get an effect of that cause. I
will have the students think pair share about different causes and effects
and give me a thumb up when they have three.

Describe what instructional strategies you will use to

model/explain/demonstrate the knowledge and skills required of the
objective. (cite theories/theorists)
I will use scaffolding during my lesson I will give my students lots of
support and show them several examples of how to use a function table
and then I will include them in practice. They will then be able to practice
examples on their own with help from me if necessary. This is a practice by
Vygotsky. I will also use an anchor chart of the function table that I will
make during my lesson. The students then can look at this hanging in the
classroom for additional support when needed. My biggest goal is to get
my students engaged and to talk out how to do the problems to better
understand them. The person doing the talking is the one doing the
learning.

Describe how you will check for students understanding before moving on
to guided practice.
I would give them each a problem to do and then I would walk around and
check their progress/ answer.
We Do
(Teacher engages students in guided practice)
Describe the learning activities you will use to provide students multiple opportunities to
practice the skills and content needed to meet the learning objective(s).
I will give the students time to be with a partner and work on problems
always encouraging them to talk out loud and almost teach the other
student. I am a firm believer that they have to talk and practice to learn. I
will also have them participate in a function table smartboard activity all
taking turns and each student getting a turn. I will have a positive
environment in my classroom and if a student is stuck on a problem they
can phone a friend. I think this is good because it gives the students to
help teach one another rather than me all the time. I will always be around
to help and giving prompt and guiding when necessary. We will also do
problems on the board together with me working them out but they choral
respond the next steps. After the activity on the smart board and all of the
other practice I would explain to them that the numbers on the function
chart can be graphed and I would show them the website and we would
graph our function chart example on the website.

Describe how you will check for students understanding before moving on to independent
practice.
I would walk around and make sure that when the students are working
together to stop and have them explain it to me as well as the other
student.
You Do
(Students engage in independent practice)
Describe what the students will do to independently practice the knowledge and skills
required by the lesson objectives? (this is the post-assessment)
The students will use a function chart and an equation like 5+x=y to find
the pattern. 1-5 will be provided as x and they need to find each coordinate
for x and y and then graph them. They will then copy and paste the chart
and then explain the pattern on a separate word document. I will be
walking around to answer questions and help if necessary.

APPENDIX: Include a blank copy of your post-assessment.

Is your BLANK COPY of the post-assessment included at the end of this
lesson plan?
My post assessment is at the end of this document.
Lesson Closing
Describe how you will reemphasize the lesson objective and any
skills/content that were taught in an interactive manner (whole/small
group, etc.).
We have come very far when it comes to number patterns and you all have
done a great job with function tables and graphing your data. We will work
more on number patterns and learn how to use subtraction in respect to
number patterns and function tables next week.

ANALYZE
(This portion may only be done after the post assessment is
collected/scored.)
Describe the results of the Post-Assessment and be sure to address the following:
Students progress from pre-to-post assessment. (if applicable)
Factors that may have influenced the post assessment results.
How the results of the post assessment highlight what areas of the
lesson will require re-teaching (if any).
If applicable, insert a table/chart/graph (below) that shows the post-assessment data
results. If you used the same document for both the pre and post assessments, it is strongly
encouraged that you show the comparison.
Insert an image of your table/chart/graph here.
