Sub : Apology for being late to the ENT exam conducted on Aug 11,
2016 and request for reexamination.

I, Abbas Ali Baig, Ashraf Ali Baig, student of Davao Medical School
Foundation, batch of IMD09, was late to the ENT midterm examination conducted on
Aug 11, 2016. Id an upset stomach and I was in the washroom, I was in front of the
exam hall by 5.02 pm. I apologize for not being on time and I promise you it will not
happen again. This is my term examination and I would sincerely like to keep up my
grades this semester so I humbly request you to forgive me just this once and allow
me to take the reexamination. Its also been notified and approved at the deans
office.
