Skill Descriptor

E=4 The student consistently demonstrates a superior level of skill and execution during both
Excellent drills and game play beyond what would normally be expected for their grade level.

P+ = 3.5 The student consistently demonstrates a high level of skill and execution during both drills
and game play.

P=3 The student frequently demonstrates the skills necessary to be competent and successful
Proficient during drills and game play

B+ = 2.5 The student frequently demonstrates the skills during drills but only occasionally
during any type of game play.

B=2 The student occasionally demonstrates the skills specific to the activity during drills and games.
Basic

NY+ = 1.5 The student sporadically demonstrates the skills specific to the activity during drills but rarely
during any type of game play.

NY = 1 The student rarely, if ever, demonstrates the skills specific to the activity during any drills and
Not Yet games.

I+ = 0.5 The student is mostly unable, or refuses to be engaged in class during skill development and
therefore rarely displays any type of competence in skill acquisition.

I=0
Incomplete The student is unable to, or refuses to engage in class during skills development and therefore,
there is no evidence to assess the students progress.

Excellent benefits of living a healthy, active lifestyle. Is goal-oriented, and is actively engaged in physical
activity with a school or community program.

P+ = 3.5 The student consistently demonstrates a high level of skills, knowledge and understanding of the
benefits of living a healthy, active lifestyle. Is goal-oriented, and is actively engaged in
physical activity with a school or community program.

P=3 The student frequently demonstrates competent skills, knowledge and understanding of the
Proficient benefits of living an active, healthy lifestyle. The student frequently engages in a moderate
amount of physical activities outside of the classroom.

B+ = 2.5 The student demonstrates competent skills, knowledge and understanding of the benefits of
leading an active, healthy lifestyle. The student occasionally engages in a moderate amount of
physical activity outside of Phys Ed class.

B=2 The student occasionally demonstrates competent skills, knowledge and understanding of

physical activity outside of the classroom.

NY+ = 1.5 The student sporadically demonstrates basic skills, knowledge and understanding of the benefits

I+ = 0.5 The student is mostly unable to, or refuses to engage him/herself in the acquisition of knowledge
and understanding of health benefit topics and rarely demonstrates an active lifestyle.

Incomplete The student is unable to, or refuses to engage in learning and therefore, there is no evidence to
assess the students progress.
Cooperation

E=4 The student consistently demonstrates an outstanding level of fair play, leadership,
Excellent teamwork, cooperation, initiative, a positive attitude, and strives to lead others to do the same.

P+ = 3.5 Thestudent consistently demonstrates an excellent level of fair play, leadership, teamwork,
cooperation, initiative, and a positive attitude.

P=3 The student demonstrates a high level of fair play, leadership, teamwork, cooperation,
Proficient initiative, and a positive attitude.

B+ = 2.5 The student demonstrates a competent level of fair play, leadership, teamwork,
cooperation, initiative, and a positive attitude. Rarely, some negative behaviors arise but are
easily corrected.

B=2 The student demonstrates a basic level of fair play, leadership, teamwork,
Basic cooperation, initiative, and a positive attitude. Sometimes negative behaviors occur, but are
eventually corrected.

NY+ = 1.5 The student demonstrates a basic level of fair play, leadership, teamwork, cooperation and
initiative. Several times, negative behaviors occur, and there is some difficulty in
correcting this behavior.

NY = 1 The student rarely, if ever, demonstrates a basic level of fair play, leadership, teamwork,
Not Yet cooperation, and initiative. Often. negative behaviors occur, and correcting this behavior
is difficult.

I+ = 0.5 The student rarely, if ever demonstrates a basic level of fair play, leadership, teamwork,
cooperation, and initiative. Negative behavior is frequent and is not corrected without
removal from activity.

I=0 The student is unable to, or refuses to engage in learning and therefore, there is no evidence to

Excellent The student never demonstrates unsafe behavior, and is able to help others who are acting in an
unsafe manner or whose effort is lacking.

P+ = 3.5 The student consistently demonstrates a high level of effort, safety, and/or self-control.
The student never demonstrates any unsafe behavior.

P=3 The student demonstrates a competent level of effort, safety, and/or self-control.
Proficient Rarely, minor unsafe behavior can occur, but is recognized and corrected by the student before
the teacher needs to intervene.

B+ = 2.5 The student demonstrates a basic level of effort, safety, and/or self-control. Rarely, minor unsafe
behavior occurs, but is recognized by the student and is easily corrected by the teacher.

B=2 The student demonstrates a basic level of effort, safety, and/or self-control.
Basic Rarely, unsafe behavior occurs, but is recognized by the student or easily corrected by the
teacher.

NY+ = 1.5 The student demonstrates a poor level of effort, safety, and/or self-control. More often,
unsafe behavior occurs, or it is not recognized by the student and there is some negative
reaction to the correction by the teacher.

NY = 1The student rarely, if ever, demonstrates a basic level of effort, safety, and/or self-control
Not Yet Unsafe behaviors occur more often, it is not recognized by the student, and there is negative
reaction to the correction by the teacher.

I+ = 0.5 The student rarely, if ever, demonstrates any effort, safety, and/or self-control. Or, when an
unsafe behavior occurs, the student refuses to recognize or correct the behavior.

I=0
Incomplete The student is unable to, or refuses to engage in learning and therefore, there is no evidence to
assess the students progress.
