PROMO MECHANICS:
1. The Convert-to-Installment Facility (CTI) is open to EastWest Credit Cardholders (Cardholders) in good credit standing*.
2. The CTI Facility is made available to Cardholders from January 1 to February 28, 2015.
3. Under the said facility, the Cardholder has the option, subject to EastWests approval, to convert his/her new retail or straight charge transaction (contained in a single charge slip) to installment. New retail
transactions may consist of local and foreign retail purchases, online purchases and mail-order transactions forming part of the Cardholders latest Statement of Account (SOA) and/or the Cardholders
current unbilled retail transactions. Cash Advance transactions, monthly amortizations of other installment programs and transactions already forming part of the Cardholders revolving balance are not
eligible for the CTI Facility.
4. The minimum amount eligible to be converted to installment is Php5,000 and must be within the Cardholders available credit limit.
5. To apply for conversion, the Cardholder must fill-out and submit the CTI application form through fax numbers 830-8900, 830-8911, e-mail at cards@eastwestbanker.com or send to EastWest Call Center
Support at 38/F PBCom Tower, 6795 Ayala Avenue corner V.A. Rufino Street, Salcedo Village, Makati City.
6. All CTI applications must be received at least five (5) banking days prior to the Cardholders latest payment due date.
7. Upon approval of the CTI application, the fixed monthly installment amount will be billed to the Cardholders EastWest Credit Card account on his next and succeeding billing statements until the CTI
availment is fully paid.
8. Cardholder must allow at least five (5) banking days for processing of the CTI application.
9. EastWest has the sole discretion to approve or reject the Cardholders CTI application and, if such application is denied, EastWest has no obligation to furnish the Cardholder a notice or reason of such
rejection. EastWest also has the sole discretion to approve only a portion of the installment amount applied for. In case the conversion application is denied in full or in part, applicable finance charges, if
any, shall be imposed.
10. The Cardholders EastWest Credit Card account must be active and current. Should the Cardholders EastWest Credit Card account be suspended, cancelled, or not renewed or should the Cardholder fail
to make payments or otherwise breach or violate the Terms and Conditions Governing the Issuance and Use of EastWest Credit Cards, all of the unpaid CTI availments as well as the entire outstanding
balance in the Cardholders EastWest Credit Card account shall immediately become due and demandable. In case of partial payment by the Cardholder of the installment amount, the unpaid balance
shall incur the usual finance charges charged on revolving accounts in accordance with the Terms and Conditions Governing the Issuance and Use of EastWest Credit Cards. If the Cardholder pays the
installment amount for the promo in full per month, but has an outstanding or revolving balance on his/her previous/other purchases, said installment amount under the promo shall likewise incur finance
charges. Any and all finance charges already imposed upon the Cardholders total outstanding balance or any part thereof to be converted into installment, whether the same consists in the Cardholders
previous balance and/or new purchases/transactions, will not be adjusted, refunded or credited back to the Cardholder upon approval of the CTI application.
11. Terms and conditions on CTI Facility indicated in this form shall apply.

Cardholders in good credit standing are those whose accounts are not delinquent, not under investigation due to suspected fraudulent activities, those whose EastWest Credit Cards are not reported lost or
stolen and those who have not otherwise, violated any of the Terms and Conditions Governing the Issuance and Use of EastWest Credit Cards.

In signing this Convert-to-Installment Application Form, I understand and agree that: (1) It is based on my instructions and has no implication on my relationship with any establishment or card issuer; (2) I shall pay
the fixed monthly installment indicated above for the whole payment term I chose which fixed monthly installment shall form part of the total statement balance on my EastWest Credit Card; (3) I may opt to pay
only the Minimum Payment Due on my total statement balance wherein only a percentage of my monthly installment is paid, in which case I will be imposed a finance charge in addition to the installment interest.
If I pay the full amount of my monthly installment but an outstanding balance on my previous or other purchases still exists in my total statement balance, I shall also be liable to pay a finance charge in addition to
the installment interest; (4) In case my EastWest Credit Card is suspended, revoked or cancelled, all my unbilled monthly installments shall immediately become due and demandable and form part of my total
outstanding balance; (5) In case of pre-termination, I agree to pay an Installment Pre-termination Processing Fee equivalent to 5% of the remaining principal balance or Php500, whichever is higher; (6) I shall
continue to be bound by and comply with the Terms and Conditions Governing the Issuance and Use of EastWest Credit Cards which are incorporated herein by reference and of which I was furnished copies by
EastWest as well as the Terms and Conditions and Promo Mechanics of this Convert-to-Installment Application.
