3. CERTIFICATION OF RESTRICTIONS ON LOBBYING
This certification is a material representation of fact upon which reliance is placed when this
transaction was made or entered into. Submission of this certification is a prerequisite for making
or entering into this transaction imposed by Section 1352, Title 31, U.S. Code. Any person who
fails to file the required certification shall be subject to a penalty of not less than $10,000 and not

4. GOVERNMENT-WIDE DEBARMENT AND SUSPENSION
(NONPROCUREMENT)
This contract is a covered transaction for purposes of 49 CFR Part 29. As such, the contractor is
required to verify that none of the contractor, its principals, as defined at 49 CFR 29.995, or
affiliates, as defined at 49 CFR 29.905, are excluded or disqualified as defined at 49 CFR 29.940
and 29.945.
The contractor is required to comply with 49 CFR 29, Subpart C and must include the requirement
to comply with 49 CFR 29, Subpart C in any lower tier covered transaction it enters into.

The certification in this clause is a material representation of fact relied upon by the Central Ohio
Transit Authority (COTA). If it is later determined that the bidder or proposer knowingly rendered

7. DECLARATION REGARDING MATERIAL ASSISTANCE/ NONASSISTANCE
TO A TERRORIST ORGANIZATION
This form serves as a declaration of the provision of material assistance to a terrorist organization
or organization that supports terrorism as identified by the U.S. Department of State Terrorist
Exclusion List (see the Ohio Homeland Security Division website for a reference copy of the
Terrorist Exclusion List).
Any answer of yes to any question, or the failure to answer no to any question on this
declaration shall serve as a disclosure that material assistance to an organization identified on the
U.S. Department of State Terrorist Exclusion List has been provided. Failure to disclose the
provision of material assistance to such an organization or knowingly making false statements
regarding material assistance to such an organization is a felony of the fifth degree.
For the purposes of this declaration, material support or resources means currency, payment

1. Identify the communication protocols and networking requirements that are required
for implementation and operation of the proposed system. In the event that there are

been provided but should be supplemented or revised by Proposers based on their
