This letter serves as an officiat complaintpertaining to the above referenced

fBcility and
former patient as follows: elder abusen dehydratiLn, undiagnosed
fraeture of the
spine, physical and verbal abuse, unexpliined bruising Jn auoom"n, talse
charting in medic_al_records, anti-psychotics given witfiout prioi consent, lack
of
dignity and insutficient staff to meeithe needi of the patient.

An Ms. Zizzo had a fall at home, Her daughter called g11 and she was sent to
1212112,
Kaiser HospitaURoseville emergency room and diafinosed with a
bruised back with no
frastures showing ol tl',e x-rays. Kajser doctors adiisea that she be ptaced in nursing
home for approximately one week of rehabilitation.

pV evening on 1il2112, Ms. Zizzo was sent to Roseville Point Health and welness
center. on 1213112,.h.er daughter, Marisa conover, the holder
of her mother,s Durable
Power of Attorney,.while.signing the-admissions paperwork speciticatty
intormeo
Rebecca Holland, the Admissions Director, that hei mother was
not td ne given any
antipsychotic medications. She was reassured that her mother would
not 5e givenLny
antipsychotic drugs. Ms. Conover also inforrned the facility that hei mother
was not
incontinent and a diaper was unnecessary and that she should
be taken to the
hathroom. However, within a day of admittance she was put in diapers

Qn 1216112, Ms. Zizzo was in such h.orrific pain and seemed drugged
that her daughter
contacted her rnother's regular physician, Dr. Keith Katsura, ano-n'e
iujgesteO that she
be transported to Kaiser/Rosevitte-EH foiturther examination.
She *ai giren a CT
scan which showed no head injury. Ms. conover asked Dr. williamsto
conouct further
tests on her back because her level of pain was so acute that there must
ue so*etr,lng
wrong. Dr. Williams refused to do any further tests stating that there no iracture
as
noted on the x-rays taken on 1Z/Zl1B-so sfre *as seni badk to Roseville point.
