CONTRACT OF SERVICE

KNOW ALL MEN BY THESE PRESENTS:

This CONTRACT OF SERVICE entered into CAGAYAN DE ORO CITY by and among:

The ERJAL/ARAH TRADING with office address located at Leandro Neri st. Zone 2,
Bayabas, Cagayan de Oro City, represented by MR. ALBERT OWEN T. TAAL, owner, hereinafter
to as the FIRST PARTY , and

________________________________, Filipino, of legal age and residing at

____________________________________________ hereinafter refered to as the SECOND
PARTY .

WITNESSETH

1. That the First Party is in need of the services of the Second Party who shall perform work
accordingly;
2. That the Second Party has signified his intention, to which the First Party has accepted, to
provide the services needed by the latter;
3. The Second Party hereby posses the experience and skills to perform the job as described
herein;
4. .That in view hereof, the Second Party is hereby contracted as an PROMO DICER for the
period of _________________________ to _______________________, in consideration
of a monthly salary of SIX THOUSAND EIGHT HUNDRED SIXTY FOUR (Php 6, 864.00) to
be paid on QUISENA or every 15 days basis.
5. That the Second Party is to work 8 hours/day, 24 days in a month with once a week day-
off, and NO WORK NO PAY policy;
6. That the Second Party is entitled of a 30% additional in the salary when working on
SPECIAL HOLIDAY, and DOUBLE PAY working on REGULAR HOLIDAY;
7. That the Second Party is given a Sale Quota of EIGHTY THOUSAND PESOS (Php 80,
000.00) per month;
8. That the Second Party is entitled of the benefits such as PhilHealth, SSS, and PAG-IBIG;
9. That if the Second Party, is about to resign from the First Party-Establishment, submission
of RESIGNATION LETTER must be ONE (1) MONTH before the effectivity of the
resignation date.
IN WITNESS HEREOF, the parties hereto have hereunder affixed their signatures this ______ day
