ACKNOWLEDGEMENT

Nothing that is worth having comes easy.

A simple yet meaningful quote that helped and motivated us as we


worked on this clinical paper. Though we are not the experts in
making clinical papers, there may be a lot of challenges and
hindrances along our way, we believed that with team work, hard work
and perseverance, we can have a productive and informative output. As
we go along our way in making this paper, God sent us angels that
helped us to make this clinical paper possible. At this point, allow
us to give them merit with our simple appreciation.

First, we would like to thank God our loving father, who guided as in
whatever we do and wherever we go as we are working this clinical
paper and in the clinical area. We thank you lord for giving us this
opportunity and for giving us the presence of mind that we were able
to reach where we are now.

Second, we like to thank our parents who gave us support may it be in


moral, spiritual, financial and in any other forms. We thank them for
sending us to school and giving us the opportunity to learn and for
letting us explore what our capabilities are.

Third, we thank Saint Paul University Dumaguete, College of Nursing,


for molding us to be a Christ-like professional Paulinian Nurses and
giving us the opportunity to integrate what we have learned in the
four wall of our classrooms.

Fourth, we would like to thank Ms. Malou V. Dinopol RN-MAN, our dean,
for the endless support and for always including us in her prayers.

Lastly, we thank Miss Gerah P. Valle RN-MAN, our clinical instructor


and serves as our second mother who endlessly extend her patience to
us. We would like to thank you miss for the knowledge that you
imparted to us which we can use wherever we are in the clinical area.
For the support and praises with the simple procedures that we
performed. We thank you miss for letting us practice the things we
wish to do for our future.

ABSTRACT

This is a case of our patient with the initials of J.P.B a 82


year old male was referred from Bayawan District Hospital to Negros
Oriental Provincial Hospital for further examinations last February
17,2017 at 4:07 pm with a chief complaint of Dili na siya makastorya,
makagimok ug makakaon ga sige nalang siyag tulog as verbalized by the
significant others. The patient had prior episode of stroke back in
2013 and 2015. He is diagnosed with Cerebrovascular infarct, re-
stroke. During his admission, he is bedridden with whole body
paralysis.

He was received by Dr.H given a diagnosis of CVA infarct, re-


stroke and was ordered PNSSiL at 15gtts/min to be given. He was
ordered for CBC, urinalysis, CT-Scan, chest x-ray, blood typing to
know the blood type of the patient with a result of blood type A+ and
be given immediate action whenever complications or further
impediments arise. Patient was instructed to be on an NPO status but
was inserted with NGT on March 1, 2017 but was removed because of
presence of clogging and was re-inserted on March 2, 2017.
