2. I have empolyed Arif Hussain S/o Ghulam Shabbir as a full
time qualified person at above mentioned Pharmacy and
registration No is 5218-A/09.
3. I am not employed is any Govarnment/ Semi Government
autonomous institution.
4. I have not been convicted from any court of law in pakistan.
5. I am physically and mentally fit for this Job.
6. I undertake that I shall not purchase / store sell any expired
and spurious Drug or any other Drug in violation to any
provision of Drug Laws at any premises moreover, I shall not
coordinate / help any pesrson found involved in the sale of
expired and spurious Drug.
7. I shall abide by the all previous of drug Act 1976 Rules
framed their under.
