Worcester County Chapter of APICS 2017 Academic Award.

The Worcester County Chapter of APICS will make available one academic award per year in the
amount of $500 to family members of Chapter members. Applications shall be due to the
Chapter Academic Liaison by Monday, 1 May 2017. Any awards will be announced by 1 June
2016. Awards will be made from the Chapter directly to the recipient by check or other means
of transfer. No person may receive more than one award. The Board will make no more than
one award per year. The Board is not required to make an award if there are no applications or
if the Board does not feel an award would be appropriate given the applications received.

Eligibility.
1. The applicant must be an immediate family member of a Chapter member, viz. child or
spouse. The Chapter member must have been a member of the Worcester County
Chapter for at least one year prior to the award.
2. The applicant must be enrolled as a full time degree student (associate, bachelor,
masters or doctoral) at an accredited college or university.
3. The applicant must have at least six months remaining in his or her degree program
following the award.
4. The applicant must submit a complete application within the time frame specified
above.
Application.
1. All parts of the application will be typed and printed on letter or A4 size paper, in 12 or
14 font size, and in Calibri or Times New Roman font.
2. Demographic and eligibility information
a. Name
b. Address (where any award should be sent)
c. E-mail address and phone number
d. Name and location of college or university
e. Degree program including degree, major field of study, length of program and
students remaining time in the program.
f. Name of the Worcester County Chapter of APICS member who is sponsoring the
applicant.
3. Supporting documentation
a. No more than one page statement (1.5 or double spaced) by the student
explaining how the award will contribute to the completion of his or her degree.
b. A letter of recommendation from one faculty member or administrator at the
college or university.
c. Recent (within 3 months) transcript from the college or university (good quality
copy is all right).
d. A statement that the student meets the eligibility requirements for the award.
