Accomplishment Activities

July 26-28, 2016

First week of our training, at exactly 7:20 in the morning I arrived in the
office of Pampanga Action Team (PAT) Region III. Maam Fe C. Evaristo,
SWO II, oriented us about our duties and responsibilities being a Social
Work student trainee. Then after she discussed the orientation about,
she divided us into 2 groups. Afterwards, maam Evaristo introduced
Sir Carlo Renzo Torres, SWO II, and the other employee in the office.
I was assigned to encode the different intake sheets form from
different districts seeking educational, medical assistance.
I was tasked to make an assessment, with clients forms, seeking
financial and medical assistance.
I had reviewed all the files of General Intake Sheets forms for E-AICS
documents from different districts with different barangay.

August 1-4, 2016

The second week of our training, I was assigned to encode the clients
forms seeking educational and medical assistance, and I had encoded
(198) forms.
Filled out and checked their attached requirements if it is completed or
incomplete from the different barangays.
We were tasked to review the attached requirements for educational
assistance.
Interviewed clients who need financial assistance.
Assessments for College and High School Students.
Reviewed the following attached documents whos seeking financial
assistance
Encoded financial assistance from different barangays.
Reviewed the attached documents of clients. Seeking educational
assistance.

II. PROBLEM PRESENTED

The subject was diagnosed to have SQUAMOUS CELL CANCER OF THE CERVIX
STAGE IIIB last January 2016. She needs to undergo 25 sessions of radiation
therapy, 6 cycles of chemotherapy, and 4 sessions of brachytherapy including
other laboratory test that will cost Two Hundred Sixteen Thousand Two Hundred
Fifty Pesos (P216,250.00), However, the Subject cant afford to shoulder her
expenses, because the family has only limited source of income, which they
cant provide the needed of the subject.

III. CLIENT AND FAMILY BACKGROUND

The Family

Alfredo Timoteo and Redilla P.Timoteo are the biological parents of the subject
and are both natives of Mexico, Pampanga. They were able to finished Secondary
Education in different schools. They were blessed with (6) six children and all of
them have their own family.

The eldest is Rolando Timoteo, presently working as a driver in a Private


Company, and he was married to Dominga and they were blessed with (4) four
Children. The second child is Willy Timoteo, he was married to Beng with (3)
three children. He is presently sick and jobless. The third child is the client. The
third youngest is Reggie who works and lives with his family in Canada. He is
extending minimal amount to the client for her medication. The second is Juvy,
he works as Construction Worker and presently married to Editha and they were
blessed with (4) four children. The youngest is Miriam who is a plain housewife
and married to Alberto who works at Electric Power Plant and blessed with (3)
children.

They raised their children by supporting them from being farmers. They are able
to provide the needs of their children with their irregular income. The family
supports one another when someone is in the midst of crisis, but they can only
extend minimal help because they have their own family to support.

THE CLIENT

The subject is 51 years old, an AB College Graduate and native from Culubasa,
Mexico, Pampanga, where she met her common law husband Marcelino Sambili.
Presently, client live at Purok 2, San Isidro, City of San Fernando, Pampanga ,
renting a house for (10) for almost Ten years now, it is made from light concrete
materials with s(2) bedrooms, they were accept the fact that even they dont
have a children due to her health condition. Her common law husband works as
a street vendor selling hot pandesal every morning where he can earns P 350. 00
a day, which insufficient for their daily needs and other expenses, including
house rental that cost Two Thousand Five Hundred Pesos (P2,500.00) a month.

On year 1982 when the subject worked as Librarian at Republic Central College,
Angeles City. After four years, she met her partner Marcelino who worked in a
Bus Company as conductor. Since then, client and his partner decided to live
together, she engages herself as plain housewife unfortunately, after few years
Marcelino find unemployed, while client is suffering from diabetes mellitus and
need to sustain her medication.
After a years past by couple had a difficult situation in their lives but never give
up and have strong faith in god. Client is very thankful because her partner
Marcelino is very supportive and understanding partner. He doing the entire
household chores even he is tired coming from his work as a vendor. Because
marcelino knows that his partner needs more attention a caring, he do
everything just to feel his wife happy, despite of her illness, also with the help
with their relatives, friends and neighborhoods and others led them to stay
strong.

Accordingly, prior to her diagnosed SQUAMOUS CELL CANCER OF THE CERVIX


STAGE IIIB, she is already suffering from Diabetis Mellitus for 3 years. But last
January 2016, she felt unusual and weakness feelings like dizziness, easily get
tired from simple work, but she ignored it because she only thought that this
only a part of her sickness with Diabetis Mellitus, but her condition got worst
every day, until she decided to consult with Doctor Claudine Marquez last May
27, 2016 because of vaginal bleeding. Since then, a month after, she makes a
second opinion. But same findings with the first Doctor. Then after (2) days
together with her partner decided to see Dr. Gaddi, an OB Gyne at St. Ferdinand
Medical City, City of San Fernando, Pampanga, for 3 rd opinion and was
diagnosed with SQUAMOUS CELL CANCER OF THE SERVIX STAGE IIIB.

Now her partner, never stop in looking other possible resources to help her in
sustaining the needed expensive treatment, and also her family has limited
income in providing the amount needed of the patient medications.

IV. ASSESSMENT

Presently, the client is financially and emotionally suffering in providing the


needed amount of the subject medical treatment. Source of income of the family
is just enough to provide their basic needs. They seeks financial assistance to
their relatives and friends in able to manage her medication, but still not enough
for the cost of her treatment which is Two Hundred Sixteen Thousand Two
Hundred Fifty Pesos (P216,250.00) a month. Now the family is looking for outside
interventions and other possible assistance to support the subject needs that
can continuous her medical treatment.

Based on the aforementioned facts, the undersigned worker recommends Mr.


Marivic Paras Timoteo to avail financial assistance from DSWD Regional Office III to
sustain his needed medical expenses.

II. PROBLEM PRESENTED

Jaypee P. Sicat is seeking financial assistance for his lifetime medication. The
subject was suffering from Hypertension prior to his diagnosed case of End Stage
Renal Disease Secondary to Hypertension Nephropathy and he needs a regular
Hemodialysis three times a week that costs Forty Nine Thousand Eight Hundred
Sixty Pesos (P 49,860.00) monthly.

III. CLIENT AND FAMILY BACKGROUND

Client

Jaypee is a native from Mabalacat, Pampanga. He is the second youngest among


the four (4) siblings in the family. He is living with his wife and his parents in their
ancestral house in Mabalacat, Pampanga. Their house is made of concrete materials
with (2) two bedrooms, given by the National Housing Authority (NHA).

In 2000, he took up Computer Science at Holy Angel University. Then, after he


graduate, he took another four year course of (Bachelor of Elementary Education) at
Republic Central College (RCC), in Angles City. Presently he is practicing his
profession as Public School Teacher at Mauaque Elementary School for (6) years as
a public school teacher, he receives a net pay of Six Thousand (P 6,000.00) a month
from his basic salary of Nineteen Thousand Pesos (P 19,000.00) because of his
several loans.

Prior to the discovery of his illness, client serves his profession with full of life and
joy. He is very active, responsible and also very supportive teacher to his students
and dynamic on the sports competition. On clients standard of living he never
thought that he would be a patient and diagnosed with End stage Renal Disease
secondary to Hypertensive Nephropathy that need to undergo lifetime medication.
Accordingly, client believed that he gain his illness when he work at call center
where he used to impede his personal necessity specially when using a restroom to
take a pie but then they has a family history with hypertension from his paternal
kin. After few years, client felt unusual feeling and he believe that he is just suffered
from hypertension and was to late when he found out that his creatinine level shoot
up, an indication kidney breakdown. The families get anxious for his condition, but
instead to show up their frustration they built positive outlook in order to boost his
confidence where about to crash.

It was year 2010, when the client has been diagnosed for irreversible illness at
Angeles University Foundation Medical Center and presently undergoing
hemodialysis thrice a week. Due to his health condition, client has difficulty in
providing his medical expenses for his treatment. That despite of client permanent
status on his work as Professional Elementary Public teacher its stresses his
condition and seeking outside resources from his friends and colleagues but still
not enough to support his regular medication.

Family

His father who is already in senior age extends support for him from being a school
gardener with monthly salary of Php 5,000 that helps client in his weekly expenses
for his dialysis but still not enough to sustain his medication as well the other
expenses including of his family needs while his mother is a fulltime house wife.

Client second eldest brother namely Chistopher Sicat is married to Josephine who is
a housewife and blessed with a child. He worked as a public teacher with a
minimum wage. His sister Almira is married to Aldrin Izon and blessed with a child
while his eldest sister who pass away due to brain cancer is separated with his
common law husband and bless with a child.

His wife used to cooked food and sell with the school teachers were client worked,
in which she earn Php 100 a day and free meal for her family in able to help client
in their daily needs.

The family extend support with the client but since they lived with their own life
and earning insufficient income in providing their family needs they find difficulty
for the ample amount for the medication of the client.

Client and his relatives have a good relationship. They lived in a resettlement area
which near at the market, school, church and different establishments that
accessible for every family needs. The mode of transportation is also accessible for
everybody. Prior to their relocation, their community is suffer from Mt. Pinatubo in
year 1991, their properties and sources of livelihood are taken away by the blitz of
volcanic wreckage that entitles his parents to grant a housing relocation where he
live.

V. ASSESSMENT

Jaypee P. Sicat needs to undergo a regular Hemodialysis three times a week that
costs Nine Thousand Pesos (P 9,000.00) to include his medicines and other
laboratories can cost Forty Nine Thousand Eight Hundred Sixty Pesos (P 49, 860,
000.00) a month. At present he used his Philhealth where he can avail discount
amounting to Two Thousand Six Hundred Pesos (P 2,600.00) per session. The family
extends support to the client but since they have a own family to prioritize.. While
client receives only six thousand pesos (Php 6,000) from his monthly salary of
Nineteen Thousand Pesos (P 19,000.00) because of his several loans which is very
insufficient to manage his medication expenses. Now the family is looking for
financial assistance and other resources to support clients needs that can
continuous his lifetime medication.

VII. RECOMMENDATION
