AFFIDAVIT OF SELF-ADJUDICATION

I, LITO T. ERASMO, of legal age, Filipino, and a resident of Bacolod City, after having
been duly sworn to in accordance with law, depose and state:

1. That I am the only son of WILFREDO ERASMO who died without any will in
Victorias City;

2. That at the time his death, my father had no debts, liabilities or obligations to any
persons, agency or institution;

3. That the only properties he left are two parcels of land consisting of 283 square meters
(each) located in Victorias City more particularly described as follows:

TRANSFER CERTIFICATE OF TITLE NO. T-227507

A parcel of land (lot 5, block 10) situated in the municipality of Victorias province of
Negros Occidental. Bounded on the NE by lot 2; on the SE by lot45; on the SW, by road lot 7;
on the NW by lot 30-B-l; and on the NE by lot l, consisting of an area of two hundred eighty
three (283) square meters, more or less xxx

TRANSFER CERTIFICATE OF TITLE NO. T-227622

A parcel of land (lot 6,block 10) situated in the municipality of Victorias province of
Negros Occidental. Bounded on the SE by lot 7; on the SW, by lot 7; on the NW by lot 30-B-l;
and on the NE by lot 5, consisting of an area of two hundred eighty three (283) square meters,
more or less xxx

4. That pursuant to Rule 74, Sec. 1 of the Rules of Court, I hereby adjudicate unto myself
the above-described properties by means of this Affidavit and hereby files the same with
the Register of Deed of the Province of Negros Occidental with the request that the said
adjudication be made effective without judicial proceeding as prescribed by the
aforementioned provision of the Rules of Court.
