Personal Strengths
Muriel is willing to participate in any activity and has a positive mindset. She has strong
interpersonal relationships and is able to work well in social settings and interacts well with
others. She is literate, enjoys writing and is eager to learn and is open to suggestions from others.

Clients Needs and Deficits


Although Muriel has many strengths, she also possesses a variety of needs and deficits.
There are times where patient is difficult to understand and will need to repeat herself until
understood. She has short answers to open ended questions, seems to have any easier time with
closed ended questions, and does not go into depth about the open ended questions asked. Lastly,
the patient will follow her boyfriend, whom is also a part of the program at Stearns, and not
focus on activity at hand when he is around.

Leisure Interests
The client has played this for some time, and currently participates in the Special
Olympics playing baseball, softball, and basketball
In the past, the client has played board games, and has engaged in puzzles, using coloring
books, as well as reading and writing.
In the future, the client is open to dancing, listening to music, reading and writing,
coloring books, card games, cooking with simple recipes, participating in puzzles with a group,
learning to play volleyball and playing on the playground.

Barriers to Leisure Participation


A few of Muriels barriers to leisure participation is her eagerness to isolate herself with
her boyfriend and do whatever activity it is that hes doing. Her speech can make it challenging
to understand her, as she has difficulty convery or expressing words quickly, which can prolong
tasks and activities. She relies on peers to do recreational activities, and does not show initiation
to do her own activities. Her motor skills are limited, as she has difficulty moving quickly.

Socialization Skills and Abilities


Muriel seemed willing to share her personal information and had a positive attitude. She
incorporates peers in group activities and enjoys the presence of others. As mentioned above, she
has a boyfriend in the program, who she enjoys spending time with.

Leisure Resources Awareness


The playground is available outdoors, as well as balls and equipment for outdoor sports,
which the client is fond of. Board games and arts and craft supplies are also available.

Social Support System


Muriel lives with her three younger sisters in Long Beach, California. She sees her family
and friends everyday, and spends quality time with them regularly.

Medical and Medication History


The patient takes one pill per day, the type of medication is currently unknown.
Justification for Recreation Therapy
It is important for client to learn how to do leisure activities independently and gain the
confidence to initiate her own activities. Recreation Therapy will give her more ideas and outlets
for recreation that she was unaware of in the past and will allow her to engage in different
activities she has interests in.
Special Precautions and/or Contraindications for Services
Her relationship with the other clients could possibly cause behavioral issues when she is
asked to focus on an activity other than her friends. She seems to get distracted by her boyfriend
and friends in the group, so keeping her attention will be a challenge at times.

Frequency of Treatment
We will be meeting with the client every two weeks for the next two and a half months.

Treatment Goals
-Goal 1.0: To increase the clients attention span on a given task.
-Goal 2.0: To demonstrate leisure independence and initiation by the end of two months

Treatment Objectives
-Objective 1.1: The client will participate in an activity on her own for at least 10 minutes
with minimum assistance. ex. Drawing, coloring, journaling
Change 10 minutes to 5 minutes
-Objective 1.2: The client will be able to sit and write a personal reflection on her own for
10 minutes by the end of the second month.
The client will be able to participate on an activity on her own with minimum assistance
for 10 minutes by the end of Nov 15.
-Objective 2.1: Given a list of leisure activities, the client will verbally identify three that
she would like to participate in by the end of the second week.
-Objective 2.2: Within two months, the client will initiate and engage in a physical
activity of her choice. ex. Catching and throwing a ball
Change to leisure activity instead of physical activity she is more then capable of participating in
physical activities. Her progress is that she was able to identify what she wanted to play but we
gave her options/ choices.

Future Recommendations
We intend to aid the client in exploring leisure and recreation activities on her own
without the need of others. Although Muriel enjoys playing sports, she also seems to be
enthusiastic about writing, which should be encouraged by others. Encouraging her writing skills
will benefit her by allowing her to express her feelings and desires, which she struggles to do so
verbally.

Pt has previously participated in the special olympics for softball. Pt has been going to
Stearns Park for recreational activities for years.
Personal Strengths:
Pt can answer well to close ended questions. Pt is physically able to play sports AEB
was able to shoot hoops in basketball. Pt is open to any activities.
Presenting Problems & Needs:
Pt claims to have a boyfriend and gets distracted by him.

Pt is well aware that she plays softball for the special olympics.
Leisure Attitudes:
Leisure Attitude is positive AEB she is open to learn anything. When asked if pt is
open to learning a new skill she says yes. Pt is motivated in leisure activities AEB smiles
frequently.
Leisure Skills:
Pt has good leisure skills AEB can physically play basketball and is in softball
special olympics
Cultural/ Social Behavior:
Pt socializes well with other peers AEB smiles and hugs almost everyone she
recognizes.

Recommendations for Therapeutic Recreation (how often will the patient be seen and

Pt will be seen every other week on tuesdays for about half an hour.
