Checklist of Technical & Financial Envelope Requirements for Bidders

The Eligibility Documents shall contain the following:

CLASS A DOCUMENTS

license and registration for the type and

Valid Philippine Contractors Accreditation Board (PCAB)
cost of the contract for this Project; (VEPCO-BD-01)

Registration certificate from the Securities and Exchange Commission (SEC), Department of

Trade and Industry (DTI) for sole proprietorship, or Cooperative Development Authority
(CDA) for cooperatives; (VEPCO-BD-02)

Mayors permit issued by the city or municipality

where the principal place of business of the
prospective bidder is located; (VEPCO-BD-03)

Construction Contracts
Duly signed list of all Ongoing and Completed Government & Private
including contracts awarded but not yet started. (VEPCO-BD-04)

Audited financial statements, showing, among others, the prospective total and current assets
and liabilities, stamped received by the BIR or its duly accredited and authorized
institutions, for the preceding calendar year;

NFCC computation (VEPCO-BD-05) or Credit Line Certificate; (VEPCO-BD-06) and

CLASS B DOCUMENTS

The Technical Component shall contain the following:

Bid Security in the form of the following:
 1. a bank draft/guarantee or an irrevocable letter of credit issued by a foreign bank, it
shall be accompanied by a confirmation from a Universal or Commercial Bank; or

2. a surety bond accompanied by a certification coming from an authorized Insurance

Commission that a surety or insurance company is authorized to issue such instrument;

Duly signed construction schedule and S-curve with PERT/CPM; (VEPCO-BD-09)

Duly Signed Manpower Schedule; (VEPCO-BD-10)

Duly Signed Construction Methods in narrative form; (VEPCO-BD-11)

Duly Signed Contract Organizational

Chart; (VEPCO-BD-12)
Supported with the following:
1. Contractors Letter Certificate to Procuring Entity (VEPCO-BD-13)
2. Key Personnels Certificate of Employment (VEPCO-BD-14)

Duly
Signed Statement of Availability of Key Personnel (VEPCO-BD-15Error! Reference source
not found.) and the
List of Contractors Key Personnel; (VEPCO-BD-15-
PERSONNEL)

Supported with the following:
1. Key Personnels Biodata; (VEPCO-BD-16)
2. Photocopy of Valid Professional Licenses Identification Card and
3. Photocopy of the Certificate of Training of the Construction Safety and Health
Personnel/Safety Officer

Duly Signed Statement of Availability of Equipment (VEPCO-BD-17-1) and the List of

Contractors Equipment (owned or leased) (VEPCO-BD17-2-EQUIPMENT)

Equipment utilization schedule; (VEPCO-BD-18)








Affidavit of Site Inspection; (VEPCO-BD-19)

safety and health program duly signed by the Safety Officer and the Owner of the
Construction
company;

Omnibus Sworn Statement

The Financial Component shall contain the following:

Duly Signed Bid Form; (VEPCO-BD-20)

Bill of Quantities; (VEPCO-BD-21)

Duly Signed Detailed Estimates including a summary sheet indicating the unit prices of construction
materials, labor rates and equipment rentals used in coming up with the bid; and

Duly Signed Cash Flow by Quarter and Payments Schedule. (VEPCO-BD-22)
