Introduction

Traditionally, a community has been defined as a group of interacting people living in a

common location. The word is thus often used to mean a group that is organized around common

values within a shared geographical location. However, the definition has evolved and been

enlarged to mean individuals who share characteristics, regardless of their location or type

of interaction. It functions within a particular sociocultural context, which means that no two

communities are alike. The physical environment varies, and so with the peoples way of

behaving and coping. The people are different from each other, thus the dynamics in one

community differs from that of the other. It is made up of institutions and organizations linked in

a complex network having a formal and informal power structure and a communication system.

In addition, the core represents the people that make up the community.

Nursing is a profession that serves the needs to the society in the area of health.

According to Nightingale it is the act of utilizing the environment to assist the patients to

recovery. Nursing is a line work requiring academic and practical preparation. Virginia

Henderson state that nursing is to assist individuals sick or well in the performance of those

activities contributing to health or its recovery that he would perform undecided if he had the

necessary strength, will or knowledge and to do this in such as way to nap him again independent

as rapidly as possible. In American Nursing Association (ANA), it is the diagnosis and treatment

of human responses to actual and potential health problem.

Community Health Nursing is a specialized field of nursing practice. Its basic knowledge

and skills are anchored on nursing theories and important concepts from the science and public

health. The ANA define community health nursing and highlights the following important goals:

the goal of professional practice is the promotion and preservation of the health of populations;

the nature of practice is comprehensive, general and continual and not episodic; the base

knowledge comes from nursing and public health; the different levels of clientele were

individuals, families and population groups; and the practitioners recognition of the primacy of

the population as a whole.

Community Diagnosis, is also known as community assessment or situational analysis. It

is designed to collect data about the community in order to identify the different factors that may

directly or indirectly influence the health of the population, to analyze and seeks explanations for

the occurrence of health nursing diagnosis are then derived and will become the bases for

developing and implementing community health nursing interventions and strategies.

Our group during the immersion conducted the following in organizing the community

utilized the nursing process; the therapeutic interventions; critical thinking and profession

communication to provide caring services to clients in the community; established culturally

competent care for the community and its members, recognizing the economic and ethnic

characteristics of the community; and demonstrated therapeutic communication skills with the

clients with interdisciplinary members of the community team.

The researchers were assigned at Barangay 679 Zone 68 Baseco Port Area Manila in the

5th District of Manila. To the north, Port Area is bounded by the Pasig River, separating the area

from Tondo and San Nicolas areas of Manila. To the east, Bonifacio Drive separates the area

from Intramuros, the oldest historic district of Manila. To the south, the 25th Street separates the

area from Ermita, Manila. West of the Port Area is Manila Bay. Considered as a depressed

community, the entire barangay has received a wide range of external assistance from both

government and non-government agencies. In fact, it was identified as one of the beneficiaries of

the KALAHI program of President Gloria Macapagal-Arroyo.'

The situation of the barangay has improved enormously since it was established.

However, flooding, poor sanitation and health conditions, and the low quality of basic services,

Particularly water, remain problems that affect the well-being of the most vulnerable group in the

community-the poorest children.

Rationale

This study aims to present the researchers to the nature of community of . Brgy. 649 Zone

68, District 5, Baseco Port Area Manila. Thus, aiding them to practice their knowledge and skills

with discipline, accuracy and in logical manner like contributing to the improvement of the

condition of the locality. The community, as the center of this study provides them to do their

role in the field of health care as a responsible health nurse.

It can produce data and information that can help to determine and identify needs,

interest and problems of the community through joint efforts and cooperation among the students

and people affected. Recognizing the needs, interest and problems of the community will enable

them to plan the action needed, offer proposals, alternatives and solution and solve these

problems with unity and coherence to have a better community setting.

Community organizing is a process by which communities and organizations

work together to identify common problems and objectives, acquire and mobilize

resources, and create and implement actions to achieve their goals.

In this study, students will learn the basis of facilitating community

organizing processes in health promotion and education contexts. These include research

and evaluation of community organizing processes. Community Assessment helps understand

the community better.

The Community diagnosis serves as reflection of the current status of the marginalized

community around Manila. It is an indicator on how far the health status of every Filipino

community has gone and how far it will go.

Statement of Objectives

General Objectives

After 6 days of community exposure, the researcher would able to determine and assess

the health status, the health related problems and the possible interventions needed in Barangay

649 Zone 68 District V Baseco Port Area Manila.

Specific Objectives

To identify the health status of the community


To collect data regarding the community source of health information
To give information about health care importance in the community and how the

community people will response to these interaction.
