Philosophy of Pharmaceutical Care
Definition of Pharmaceutical Care
The responsible provision of
drug therapy for the purpose of
achieving definite outcomes
that improve a patients quality
of life.
Outcomes of Pharmaceutical Care
1. cure of a disease
2. elimination or reduction of a
patients symptomatology
3. arresting or slowing of a
disease process
4. preventing a disease or
symptomatology
Pharmaceutical Care involves the
process through which a pharmacist
cooperates with a patient and other
professionals in designing,
implementing and monitoring a
therapeutic plan that will produce
specific therapeutic outcomes for the
patient.
1. Involves 3 major functions:
identifying potential and actual
drug-related problems
resolving actual drug-related
problems
preventing potential drugrelated problems
2. Pharmaceutical Care represents not
only a change in what pharmacists do,
but also an entirely different
philosophy of practice. Its primary
objective is to improve patient
outcomes by shifting pharmacy
services from a drug or product
orientation to a patient orientation.

Medical Care

social and psychological as well
as biomedical factors that
relate to the illness experience
of the patient
2. Providers perceive the
patient as person. This
requires understanding the
patients unique experience of
illness and the personal
meaning it entails
3. Providers share power and
responsibility
4. Providers promote therapeutic
alliances which involves:
1) incorporating patient
perceptions of the
acceptability of
interventions

2) mutually agreed-upon
goals for treatment
3) a trusting and caring
relationship between
health professional and
patient
Patient-Centered Care includes the
following:
1. to engage in an open exchange
information
2. to involve patient in the
decision-making process
regarding treatment
3. to help them reach the
therapeutic goals that both the
patient and their health care
provider endorses
In providing Patient-Centered Care, the
pharmacist must be able to:
1. understand the illness
experience of the patient
2. perceive each patients
experience as unique
3. foster a more egalitarian
relationship with patients

promoting, or characterized by
belief in equal political,
economic, social, and civil
rights for all people]
4. build a therapeutic alliance
with patients to meet mutually
understand goals of therapy
5. develop self-awareness of
personal effects on patients
