Information from the Self-Assessment tool is used to develop SMART* goals related to student learning and
professional practice. An action plan, along with evidence to be collected, is developed to share with the evaluator.
The educator and evaluator collaborate to refine or revise the goals and plan as needed.

SMART* Goal #1 Student Learning


Based on the data that only 65 of my students currently have mastered Nevada Standards
G.CO.A.1 (Know precise definitions of angle, circle, perpendicular line, parallel line, and line
segment, based on the undefined notions of point, line, distance along a line, and distance
around a circular arc.), G.CO.A.1 (Know precise definitions of angle, circle, perpendicular line,
parallel line, and line segment, based on the undefined notions of point, line, distance along a
line, and distance around a circular arc.) and 8.G.B.8 (Apply the Pythagorean Theorem to find the
distance between two points in a coordinate system.), as measured by the Geometry
Fundamental Skills Assessment.
Educator Plan Action Steps: My goal is that by the end of the year, is to increase scores
by 10%.
Evidence: My students will have achieved mastery as measured by Unit Tests, Quizzes, Warm
Ups and the Semester Exam.
SMART* Goal #2 Professional Practice
In order to help my students achieve this learning goal, I will improve my practice on activating
students abilities understanding of new concepts and skills, employing students cognitive abilities
and skills, engage students in understanding what, why, and how they are learning, providing
opportunities for extended, productive discourse.
Educator Plan Action Steps: I will measure my progress toward this goal by I will improve

Evidence: I will measure my progress toward this goal by engaging in mentoring; instruct
summer school and obtaining a master's degree in education. In working in these areas of focus,
