Discharge Plan (M-E-T-H-O-D-S)

Medication

Oriented the patient and its family members about all the medications, how to take it and
the time of administration.
Informed and warn the patient and its family members about the occurence of the side
effects and that the medications given to the patient may experience adverse effect which
are undesirable and must be reported immedietly to the physician.
Reviewed the purpose, action, and benefits of taking the medications prescribed by the
physician.

Exercise

Demonstrate to the client passive and active range of motion exercises to the unaffected
areas such as head and neck exercises by tilting side to side, forward and backward, and
rotation; shoulder and elbow exercises by moving up and down, side to side and rotating
it back and forth; also arm and wrist exercises by stretching and bending the fingers.
Encourage client to consult a physical therapist after being discharged.

Treatment

Explained to the family the condition of the patient and its possible complications if not
treated properly next time.
Educated the patient the importance of drug compliance.

Patient must have the knowledge about the physical changes she can feel after discharge.
Patient must perform hers personal hygiene daily to prevent further infections especially
Health Teaching
in the genitals.
The client must report immediately to the physician if there are complications felt.

Out Patient
Orders

Instruct the client to call the physician if any of the following worsen:
Fever
Severe pain
Swelling
Bleeding
Chronic Cough
Shortness of breath
Difficulty of breathing
Chest pain

Diet

A well-balanced diet
Eat food which are low in fat and added sugar.
Eat food which are rich in vitamins (fruits and vegetables).
Limit highly refined carbohydrates (white bread, pasta, rice, soda, candy)
Encourage the client monitor and maintain a healthy weight to decrease the strain on the
joints, lose weight by cutting on extra calories to prevent arthritis from getting worse, and
eat foods that are rich in Omega-3 fatty acids that help reduce inflammation in the body to
relieve joint pain and decrease morning stiffness.

Sex

Inform client to consult with physical therapist to follow the program regimen before
resuming sexual activities.
