Symptoms of AOM
Pain
Fever (Caveat: the younger the patient, the
more likely there is to be fever.)
Decreased hearing
URI symptoms
Irritability in young children
Draining ear is pathognomonic
May be asymptomatic

Complications of AOM
TM Perforation
Mastoiditis
Meningitis
Brain abscess
Sinus Thrombosis
Sensorineural hearing loss
Labyrinthitis
Bullous Myringitis
Similar symptoms to
other AOM, but more
likely to produce
drainage
Usually painless
May affect balance
Associated with hearing
loss
90% will clear in 90
days
Antibiotics are not
indicated

COM
Bacteriology: usually
gram positive, more
anaerobes,
pseudomonas more and
more frequent
Some antibx resistance
in 40%
Rarely viral
MRSA nearly 8%
Complications of COM
Hearing loss
TM perforation
Mastoiditis
Brain infection
Balance disturbances
Cholesteatoma
PEARL:
A reading of mastoiditis
on a head/brain MRI is
very common and
almost always reflects
changes made by old
disease.
OTITIS EXTERNA
Treatment of AOE
Not common
Vast majority of COE patients could resolve
their episodes simply by stopping Q-tip use
and keeping water out of their ears
For people with ear dermatitis, consider topical
steroid, Protopic, or Derm-Otic gtts
Many of these are fungal
When is it cellulitis?
Fever
Erythema and swelling extending out to the
pinna/periauricular area
Seepage and exudate rather than frank
drainage
Pain may be very severe
Ramsay-Hunt Syndrome
1. Topical steroid
2. Antibiotic drops
3. Antifungal drops

Glomus Tumor
Cholesteatoma: epidermal
inclusion cyst of middle ear
PEARL
Current classifications of
rhinosinusitis
Rhinosinusitis
Defined as inflammation
of the lining of the nose
and sinuses from a
variety of causes.
Treating CRS
Chronic Rhinosinusitis
1. Antibiotics
4. Oral steroids
5. Surgery
Nasal polyposis
Nasal Polyps-treatment
Be aware of Samters

polyps and aspirin
allergy

an incidental finding of
little clinical significance.

Epiglottitis
Symptoms: abrupt onset of
high fever, difficulty
swallowing, sore throat,
drooling. In children,
sitting in the tripod
position. Toxic
appearance. Stridor is a
late symptom indicating
near complete airway
obstruction. Look for
thumb sign on x-ray.

Treatment of Epistaxis
Acute treatment:
pressure, pressure,
pressure!
Topical vasoconstrictors
(Afrin)
Packing
Control blood
pressure/anxiety
Leave the ice in the
freezer with the peas.
Treatment of epistaxis
If bleeding persists, or if the nose is nicely
packed and the bleeding persists, proceed with
a posterior pack. This is an excellent time to
refer!
Caveat: A patient with recurrent bleeding needs
endoscopy to rule out tumor.
Prevention of Epistaxis
HEARING LOSS
Impaired conduction of
sound wave from the
outside world through
the ear canal, tympanic
membrane and ossicular
chain.
