Nursing is a complex field that requires more than just medical skills,
but people skills and the most important crtical thinking skills. we as nurses
build on critical thinking skills as we get more experience through the field of
work. With our patients getting more complex with multicritical diagnosis, we
are forced to advance our knowledge. Even though advancing our knowledge
is important we still cannot forget our basic foundation theory such as the
Florence Nightingale, Betty Neuman, Virgina Henderson etc. Those theory
sets the theory sets the foundation and framework of nursing practice and it
is still use until this day.
During this case study, we had a ????????? year old, Mexican American
male. Who is a Navy veteran who is visiting the diabetic clinic regarding his
insulin care. Hes chief complain was that he does not know how to start his
insulin. After the visit with the doctor, we had a diabetic nurse name Jennie
to further assess and help him with his needs. First, Jennie starts with the
basic assesments and question for Mr. Duran, through her interview she was
able to get a whole picture of his living suitation and things that is going on
with his life. Mr.Duran stated that he is married and currently lives in Chula
Vista with his wife. His daughter was just murdered not long ago which made
them even more busy because they had to care for the daughters child as
well. Mr. Duran seems terrified when he talks about the insulin syringe and
also his daughter. After we got to know alittle about Mr. Durans case we
need to assess an hoslitc aspect of his life. This is where nurse Jennie comes
in to assess his needs using Orems and Neumans theory.
Betty Neumans theorys main focus is on the human needs of
protection and/or relief of stress. She is a huge believer of identifying nursing
intervention that will help relief the stress. According to Meleis et al,(2012),
Neuman adhered to the metaparadigm concept and has developed
neumorous additional concept for her model. In her work, she defeined
human being as a client or client system, as a composite of varaibles such as
physiological, psychological, scioculture , developmental and spiritual needs.
what I learned through this case study is being able to apply the theory of
betty Neuman into real life when assessing patients such as Mr. Duran. Using
Neumans theory we can collect physiological data such as any other
comorbidity that Mr. Duran might have. Psychologial data such as how does
he deal with his current illness and how does he cope with that the will need
to take insulin for the rest of his life. Sociocultural data such as how does his
culture precieves diabetes, do they believe in diabetic treatment? Spiritual
data such as do they have enough support from others, what other support
can the nurse provide or refer them to. Lastly stress, how much stress is this
disease causing Mr. Duran and what can we do to help lower his stress. Betty
Neumans theory makes me understand and learned that assessing a patient

is not just about getting to know the patient and take down their medical
history but to take a closer look at them hoslistically.
Next we have the Dorthea Orem theory, which we used to assess Mr.
Duran. I learned that using this theory we could assess Mr. Durans
knowledge about his diabetes. This gives us a rough idea of how much does
he know about his diabetes and what we need to continue educate him on.
We need to assess his diet, which could help his diabetes tremendously if he
follow the diabetic diet. And also self care, if he has the ability to take care of
himself and administer insulin as directed.
I have used the two theory in my clinical practice such as a patient
with hypertension. I assess them holistically by asking them different
relevant question then I follow with Orems theory to assess about their
knowledge of their disease and if they know about the Dash Diet. I also
assess them using Betty Neumans theory of psychological needs. often
times patient that are diagnosed with long term illness have a hard time
dealing with the treatment regimen and follow up doctor visit Besides that
they also have trouble sometimes understanding the importance of
sedentary life style changes. Overall, I think I learned a lot through those two
theory to help better assess my patient more thorugly. Even though it might
take a longer time to interview a client but we get to learn more about them.

Juan duran
An Mexican American, visiting the diabetic clinic at the VA. He is exceedingly
polite. Been in the us for more than 50 years and is a navy veteran. He
doesnt know how to start his inuslin
Jennie the nurse interviews juan . he is married, lives in chula vista in an
apartment with his wife, he speaks English while hes wifes English is limited.
They speak Spanish at home. The vision is limited so the wife has to
administer inuslin. They had one daughter who was found murdered in the
apartment parking lot less than two weeks ago, leaving mr and mrs duran to
care for her child. Mr duran seems befuddled by the insulin and syrdinge and
is stoic when he talks about the loss of his daughter. The two theory that
help assess his needs are orem and betty newmans theory. Because orem
model deal with self care and mr duran will need care for his diabetes with
his wifes help. When using orem, the nurse interacts with medical team to
plan and administer care and assist the patient and family in learning to care
for themselves. The model also includes family culture within the
environment. For neumans model idea is about the role of stress are vitally
important a patient like mr. duran. In addition , the five interacting varaiable
physiologic, psychological, sociocultural, developmental and spiritual all will
be important in his care.
Based on dorothea orems theory what addition information would jenny want
to collect , we would want to assess the duran family current knowledge of
diabetes, diet, and self care in addition she would be concern about aspect of
his environment such as family suppor and culture
Based on neumans theory what additional information jenny need to collect
physiological data, psychological data, sociocultural data, developmental
data, spiritual data, and stress
