lie within the scrotum
left testis is usually a little lower than the right
age of 5 7, if undescended, they will not function
produces
o
sperms (spermatozoa)
o
male hormones, primarily testosterone
tunica vaginalis
o
closed peritoneal sac partially surrounding the testis
o
represents the closed-off distal part of the
embryonic processus vaginalis
o
visceral layer

covers surface of each testis, except where the

testis attaches to the epididymis and spermatic
cord
o
parietal layer

adjacent to the internal spermatic fascia

more extensive than the visceral layer

o
sinus of the epididymis

slit-like recess of the tunica vaginalis

bet. body of the epididymis and posterolateral

surface of the testis
tunica albuginea
o
tough, fibrous capsule surrounding each testis
o
thickens into a ridge on its internal, posterior aspect
as the mediastinum of the testis
o
real part of the testis as compared to tunica
vaginalis (according to lec)

connect rete testis to upper end of the epididymis
normal spermatogenesis
o
can occur only if testes are at a temp. lower than
that of the abdominal cavity
o
when located in scrotum, theyre at a temp. of
about 1.5-2.5 C (lec) or 3 C (Snell, p. 131) lower
than the abdominal temperature
blood supply:
o
arteries

abdominal aorta testicular arteries

paired visceral branches of the abdominal
aorta
0.5 cm distal from renal artery
usually accompanies the ureter as it goes
to the pelvis
however, testicular arteries pass
retroperitoneally in an oblique direction,
crossing over the ureters and inferior parts
of the external iliac arteries to reach the
deep inguinal rings
they pass through the inguinal canals and
exit through the superficial inguinal rings to
enter the spermatic cords
the artery or one of its branches
anastomoses with the artery of the ductus
deferens
o
vein

pampiniform venous plexus testicular veins

plexus converge superiorly to form the right
and left testicular veins
goes to the spermatic cord, SIR, inguinal
canal, and then to DIR
separates from vas deferens and goes up
o
right testicular vein IVC
o
left testicular vein left renal vein
lymph drainage:
o
lymph vessels ascend in the spermatic cord
o
end in the lumbar or para-aortic LNs at the level of
L1
nerve supply:
o
testicular plexus of nerves

vagal parasympathetic

visceral afferent fibers

sympathetic fibers from T10 (-T11) of the spinal

cord
clinical correlation
o

Epididymis

comma-shaped structure
solid, long coiled tubes
long, narrow, firm structure
located at the postero-medial surface of the testes
where sperm will mature and gain motility
parts:
o
expanded upper end or head
o
body
o
a pointed tail
tubes can reach up to 20 feet (6 m)
o
long length provides storage for spermatozoa;
allows them to mature
o
main function: absorption of fluids

Ejaculatory duct

1 inch or 2.5 cm long slender tubes

union of duct of seminal vesicle + vas deferens
path:
o
pierces the posterior surface of the prostate
o
opens into the posterior wall of the prostatic urethra
called verumontanum or seminal colliculus
o
opening is close to the margins of prostatic utricle
prostatic secretions do not join the seminal fluid until
the ejaculatory ducts have terminated in the prostatic
urethra

Vas deferens or Ductus deferens

primary component of spermatic cord

thick-walled tube about 18 in. (45 cm) long
conveys mature sperm from epididymis to ejaculatory
duct and urethra
majority of its length is in the pelvic area
path:
o
emerges from the tail of the epididymis
o
ascends medial to it to enter the spermatic cord at
the upper end of the scrotum
o
separates from the other vessels that go up
o
it goes inside the pelvic inlet then to the side of the
urinary bladder
o
posterior to the bladder, it lies superior to the
seminal vesicle then descends medial to the ureter
and the vesicle
ampulla of the vas deferens
o
dilated terminal part
o
within the prostate
o
inferior end narrows down and joins the duct of
seminal vesicle to form the ejaculatory duct, which
drains to the prostatic urethra

Seminal vesicle

two lobulated organs that are 2 inches long

relations:
o
anterior: urinary bladder
o
posterior: rectum
o
superior: ampulla of the vas deferens
o
inferior: prostate gland; joins vas deferens to form
ejaculatory duct
o
medial: terminal part of the vas deferens
each vesicle narrows and joins the vas deferns of the
same side to form the ejaculatory duct
contributes 45 55% of the semen
o
makes it silver, grayish color
o
vit. C
o
prostaglandin

for assisting the contraction of the fallopian

tube in pushing the sperm forward
o
fructose

energy of the sperm cell

Prostate

fibromuscular glandular organ

surrounds prostatic urethra and surrounded by a fibrous
capsule
1.25 in (3 cm)
pyramid shaped or somewhat conical shaped
bet. neck of UB and urogenital diaphgram
base
o
superior
o
continuous with the bladder neck above

smooth muscle passes without interruption from

one organ to the other
o
urethra enters its center
apex
o
inferior
o
lies on upper surface of urogenital diaphragm
o
urethra leaves it just above the apex on the anterior
surface
relations:
o
anterior

symphysis pubis
separated from it by extraperitoneal fat in
retropubic space (cave of Retzius)

puboprostatic ligament
connects prostate to pubic bones

posterior

anterior surface of rectal ampulla

separated from it by the rectovesical
septum (Denonvilliers fascia)
o
it is formed in fetal life by fusion of walls
of the lower end of the rectovesical
pouch
o
lateral

embraced by anterior fibers of levator ani

structure
o
anterior lobe

in front of urethra

aglandular
o
middle or median lobe

wedge of gland

between urethra and ejaculatory ducts

upper surface is related to trigone

rich in glands

if enlarged, can cause pressure on urethra,

causing prostatism signs of urinary obstruction
benign prostatic hypertrophy
o
posterior lobe

behind urethra

below ejaculatory ducts

contains glandular tissue

in a rectal examination, it is the mass in the

male at the anterior wall of the rectum

most common area of cancer

if fine needle aspiration in anterior rectum is

needed
no pain because there is no free nerve
ending in the muscosa of the rectum
o
right and left lateral lobes

in both sides of the urethra

contains many glands

shallow vertical groove on the posterior surface

of prostate separates the two lobes

if enlarges, chances of benign prostatic is lesser

function
o
produces thin milky fluid containing:

citric acid

acid phosphatise
o
40-50% of the seminal fluid (lec); ~20% volume of
semen (Moore, p. 379)
o
secretion

makes the seminal fluid silver looking

alkaline to help neutralize vaginas acidity

sperm cannot withstand acidity of the
vagina without it
o
it is added to the seminal fluid at the time of
ejaculation
o
smooth muscle surrounding the glands squeeze the
secretion into the prostatic urethra
blood supply
o
inferior vesical artery
o

venous drainage
o
prostatic venous plexus

lies outside the prostates capsule

receives deep dorsal vein of the penis and

numerous vesical vains

drains to internal iliac veins

stimulate smooth muscle of prostate during

ejaculation
o
sacral plexus - parasympathetic

Prostatic urethra
o
o
o
o
o

1.25 in. (3 cm) long

begins at bladders neck
passes through prostate from base to the apex

becomes continuous with membranous urethra

widest and most dilatable portion of urethra
urethral crest

longitudinal ridge

prostatic sinus
groove on each side of the ridge
prostatic glands open to these grooves

depression on summit of the urethral crest
analog of uterus and vagina in females
openings of the two ejaculatory ducts on
its mouths edge

Quotes
[Your ejaculation] can land on the floor. It can land on the
bedsheet. It can land in the posterior fornix. Or it could land,
and I hope it will not land there, in the oropharynx.

Doc
DOC BANEZ CORNER

Yung mga girls ah! Ang sama ng tingin niyo sa akin.

Tarantadong Banez yan. Bastos! Biro lang si Doc Banez. Kasi 8
am yung class. Nakaka-antok.
Galactose [in milk]. Nakatikim na ako ng human milk. I like to
taste it and I tasted that and it is really sweet. But the male,
instead of galactose, we have fructose. They say that it is
sweet, but I havent tasted. Do you have it? I dont want to
taste it. Let others taste it. Males here. Have you tasted your
semen already? Not yet. Dont taste it. Pero sabi daw nila
masarap, matamis daw.
You know, my professor, my idol, Dr. Lanting, hes a urologist.
He died of prostatic (something)*. Yun yung specialization
niya, dun pa siya namatay no?
*Di ko maintindihan yung sinabi nya.
Have you did [done] a rectal examination on yourself? When
you making uh uh? You do it tonight. Palpate for your prostate.
Wala ka namang bubuntisan sa pwet!
Wala pang semen umabot sa ceiling. Kalokohan yun. Ang
nakita kong umabot sa ceiling, yung nabutas kong aorta.
Chung!
Di pa ung semen. Chu chu chu! Chu chu chu! (imagine niyo
na lang nageejaculate si Doc/yun ung ginawa niya eh :D)
