INTRODUCTION

We were assigned to Bernabe Health center, Brgy. Pulang Lupa I, Las Pinas city for our community health nursing exposure started June
21,2010 and lasted June 29,2010.

We chose patient JTC who happened to be our patient during the prenatal checkup last June 22, 2010 for our initial database as a
requirement for our CHN exposure. Currently, JTC is a 19 year-old who happened to be 36 weeks pregnant.. Our clinical instructor instructed
us to choose a client for our family assessment. This family will be our subject for case study. We chose patient JTC who happened to
be our patient during the prenatal checkupThey are living in the congested and not so quite clean environment. Given a subject, we
formulated plans and designated task to done individually. We asked her permission to be able to visit her at home.

In this family nursing case study we will present their family structure, what kind of house and home they have, how they can survive
in their everyday needs, how they able to manage their problems and how they able to solve with each health problems.

As nursing students, this case study helps us to develop our knowledge and skills in promoting and rendering nursing care to our
patient. It also helps us to identify the needs, priorities, and problems of our client in order for us to formulate a family nursing care
plan. We establish rapport with our patient in order to ensure good quality data to facilitate partnership in addressing identified health
concerns

Objectives:

General:

As nursing students, we are expected to perform a family assessment, survey, and an efficient family care; family as the main unit of
care in community health nursing, we must be able to provide an achievable intervention regarding health conditions of the family.

Specific:

 To choose a family that needs immediate interventions regarding health conditions

 To teach the said family and render nursing care of the sick, pregnant and well patients

 To prioritize the main health problems that the family needs an immediate intervention

A. FAMILY DYNAMICS, COMPOSITION, STRUCTURE

Canete family is in extended structure as JTC still lives with her parents and other siblings . According to her, their 2-storey house
is theirs, but the 2nd floor actually is pawned to them by their relative. Common languages spoken in their family were Tagalog.

Since then, the main decision-maker is the head of the family, Mr. Jaime Canete, he is also the one who decides in money matters
however in allocating the budget for foods and groceries are done by the wife.

Regarding their health interventions, they always seek help in the nearest hospital, Las Pinas district hospital especially when one of them
is sick.

D. Health assessment
The Cañete family has no remarkable severe diseases and illness from the past years. Familial history also reveals that
grandparents died of old age not by any severe debilitating diseases.

JTC currently 36 weeks pregnant with EDC of June 25, 2010 will be having her NSD at Fabella Hospital as their main hospital
because of financial matters.

E.Values, Belief and Practices

Like other Filipino families, their own values and beliefs in life are not different, they are Roman Catholic in religion, being a
god-fearing family, as a unit of the society they usually practice their faith every Sunday, they always go to church and worship god.
They entrusted the lord regarding their health, life, and problems. They always count on each other especially in times of trouble, they
never left each other’s sides because they believe that the only person that you can have a shoulder to cry on is your own family. Like
being pregnant of the 3rd daughter, Janice, in age of 19, she became pregnant, her parents are not taking away the fact for the couple
to live with them instead of having their own house and own life away from her parents.

Pregnancy can cause a positive effect in
mother especially in happiness it will
bring to the couple, however, new born
can cause another mouth to spend for
food budget or milk supply that needs a
much higher financial allocation.
A good lighting and ventilation can
prevent any falls/accidents that can
cause a health deficit in the family.
A healthy family can be possible if some
of the vices of other family members are
prevented or stopped eventually.
