JOIN AFFIDAVIT OF DISCREPANCY

I, DIOMEDISA S. VIOJAN, of legal age, Filipino, widow and a resident of 190
Jumao-as St. Zone I, Digos City, Davao del Sur, Philippines, after having been duly
sworn to in accordance with law, do hereby depose and state that;
That I am the same and identical person who caused for the execution of this
Affidavit;
That since I was young, I have always been using my first name as
"DIOMEDISA" and that it is the same first name that I have been using in all of my
documents and identification papers;
That however, it appears from my Certificate of Live Birth that what was entered
therein as my first name is DIOMEDES;
That my true first name is DIOMEDISA as reflected in my Marriage Contract;
That the name appeared in my Certificate of Live Birth as DIOMEDES SUERTE
and DIOMEDISA SUERTE VIOJAN as stated in other public records refers to the same
person;
That I am executing this Affidavit in order the attest the truth of the foregoing
facts, to apprise the proper authorities especially the Department of Foreign AffairsDavao of the same and for whatever legal purpose this may serve best;

AFFIDAVIT
I, RIO ANGELO APDIAN AFRICA, of legal age, Filipino, single and a resident of
Digos Super Highway, Digos City, Davao del Sur, Philippines, after having been duly
sworn to in accordance with law, do hereby deposes and states that:
That I am the same and identical person who caused for the execution of this
Affidavit;
That I am one of the son of MA. THELMA A. AFRICA and LOVE B. AFRICA,
born on November 11, 1989 at Kabacan, Cotabato;
That since I was young, I have always been using my middle name as "APDIAN"
and that it is the same middle name that I have been using in all my documents and
identification papers;
That however, it appears from the records of the Office of the Civil Registrar of
Kabacan City that I was registered therein together with the middle name of my mother
as AFDIAN;
That at the time I filed for the Nursing Board Licensure Examination dated last
December, 2010 the Philippine Regulation Commission recommends that I should used
my erroneous middle name and of my mother, because if I should wait for the
corrected entries it would take long and hence, I could not take the said examination;
That there were times that we attempt to correct the above-mentioned erroneous
data, however, the said facts was not yet verify though we were given a Certification
issued by the Local Civil Registrar of Kabacan City confirming that it had been
corrected;
That my true middle name is Rio Angelo APDIAN Africa and that of my mother is
Ma. Thelma Terante APDIAN as reflected with my other records;
That it is my desire to be registered with the said Office of the Civil Registrar of
Kabacan City with the correct middle name "APDIAN";
That I am the same person who appeared to be Rio Angelo Afdian Africa in my
Certificate of Live Birth and Ma. Thelma Terante Afdian as my mother is exactly the

same person with RIO ANGELO APDIAN AFRICA and MA. THELMA TERANTE
APDIAN as stated in my other public records limited to my Certificate of Live Birth;
That I am executing this Affidavit in order the attest the truth of the foregoing
facts, to apprise the proper authorities of the same and for whatever legal purpose this
may serve best;

resident of Mabini Extension, San Miguel Digos City, Davao del Sur, Philippines, after
having been duly sworn to in accordance with law, do hereby deposes and states that:
That I am the same and identical person who caused for the execution of this
Affidavit;

That I am the legitimate mother of Luisito Alcontin Ebo who was born on April 18,
1987;
That he was baptized on April 22, 1987 at Sta. Teresa De Avila Parish, Diocese
of Digos, Malalag, Davao del Sur;
That in his Certificate of Baptism following information was erroneously entered:

That I am executing this Affidavit in order the attest the truth of the foregoing
facts, to apprise the proper authorities of the same and for the purpose of having the
above erroneous information in the Certificate of Baptism of my child Luisito Ebo be
corrected as well as for whatever legal purpose this may serve best;
