Phase VI: Evaluation and Reflection (Due December 13, 2016)

In this final phase of your project you will need to use both formative and summative data to understand if your work worked!
That is to say, you will write a data-driven assessment of how your project worked out.
What was the outcome?
What worked and what did not?
What would you do differently?
How do you know the answer to these questions?
What data are you using to make these judgments?
How did you work with your mentor and your coach?
Your assessment should also include suggestions for improving the continuation of the work after you leave (Pay it Forward!).
While the evaluation of the project must be submitted in a written format, we encourage you to create a short digital story
that: defines your project and participants, describes the work that was done, and reflects on the outcomes.
