STANDARD LESSON PLAN FORMAT

Describe the Class

5th grade

15 students are in the classroom; 2 intellectual disability students, 1


student with ASD, 1 student with emotional disturbance, 2 ADHD students, 3
ELL students, 2 gifted and talented students, and 4 students who are learning
disabled.
Objective(s)

Given a set of reading materials, students will identify the main


plot, setting, and intention of the characters with 80% accuracy.

While looking at arithmetic examples from a reading book,


students will apply basic knowledge of circumference math problems with 80%
accuracy.
Procedures
1.
Introduction: The teacher will read the introduction of, Sir
Cumference and the First Round Table: A Math Adventure by Cindy
Neuschwander.
2.
Grouping: The teacher will group the students Jigsaw style with 5
students each. All 3 groups will receive a worksheet related to the math problems
at hand. The worksheet will ask how the students will measure Sir Cumferences
table, and ask how big the table is.

Assessment: Students will use various methods to decide which
round table will be used in Sir Circumferences overall table measurement.
Students will have up to 15-20 minutes to do the class project.
4.
Evaluation: Students will have 5 minutes maximum to answer the
problem, and will explain how they got the answer. They may use multiple tools
and technology to explain to the whole classroom. Teachers will give the final
assessment based on a teacher graded rubric for all groups.

Modifications

ELL students will participate by answering problems in English


with 80% accuracy.

Students with multiple disabilities will use text-to-speech


technology or electronic pointers to answer with 75% accuracy.

Learning disabled students can either use the chalkboard or a


computer to answer the problems with 80% accuracy.

Gifted and talented students will write out their own math word
problems similar to the cooperative learning lesson. They will also have the
chance to show it to their peers about how they solve the lesson if the class has
leftover time.

Students with ADHD may do active simulations of a circle, or


make creative visuals to show how big the round table is.
Assessment

The teacher will review the basic definitions of circumference,


diameter, and radius with the students.

If all students understand the lesson, the teacher will hand out a
practice math worksheet of circumference problems for the students to take home.
