At present, there is a need to improve or strengthen coordination to integrate the
various programs, projects, standards, systems, and policies in the Department of
Health. The Unified Health Management Information System (UHMIS) aims to
integrate data collection, processing, reporting and use of the information which is
necessary for improving health service efficiency and effectiveness. The outputs of
the UHMIS will assist in the management and planning of health programs.
Specifically, the development of the UHMIS will put in place standards related to
policy formulation, data collection and transmission, data analysis, presentation,
reporting and utilization.
The current patchwork of data systems will be transformed into a unified health
management information system in the Philippines. Data from various sources can
be brought together and will allow the Department of Health to assess trends in
diseases, injuries, disabilities, health service access, and deaths. All DOH
Information Systems can be accessed through the UHMIS and related databases are
stored in the DOH Central Database Warehouse. Information Systems and data
included in the UHMIS are as follows:
National Health Data Dictionary: This establishes a uniform national data set
where information can be evaluated and compared with one another. This promotes
data uniformity, availability, validity, completeness, reliability, and consistency.
Further, the use of the dictionary will ensure uniform collection and sharing of data
throughout the health sector. [Architecture Online; Current Status: Operational]
National Electronic Injury Surveillance System: On a daily basis, hospitals are
reporting injury cases to the DOH via the Online National Electronic Injury
Surveillance System Registry. The system records injuries, patterns and factors that
may have cause the injury as well as the available services, health status needs and
circumstances of the injured person. [Architecture Hybrid of Offline & Online;
Current Status: Operational]
Kontra-Paputok Reporting System: This is a regular reporting of injury cases
resulting from firecrackers from December 21 to January 5 every year. On a daily
basis, hospitals are reporting firecracker related injury cases to the DOH via the
Online National Electronic Injury Surveillance System Registry. [Architecture

Philippine Network for Injury Data Management System: An integrated
system with multi-sectoral organizational support (Philippine National Police,
Metropolitan Manila Development Authority, Land Transportation Office, Department
of Public Works and Highway, SafeKids Philippines, UNICEF, University of the
Philippines, Department of Transportation and Communication, and others) which
establishes a coordinated data management system that can link, integrate or
combine injury data from various sources or systems. [Architecture Online;

Public Assistance Information System: Funds from the Office of the Secretary
were allocated to various hospitals to serve as medical care assistance to indigent
patients and under-privileged veterans. The Public Assistance Information System
supports the operation and management of the Public Assistance Unit (PAU) in the
Central Office. All patients assessed and referred by PAU to other DOH hospitals are
inputted into the system. Likewise, the receiving hospitals verify the authenticity of
the request through the system and update the status of service(s) rendered for
monitoring, updating and generation of reports. [Architecture Online; Current

Electronic Essential Drug Price Monitoring System: This system enables
hospitals, pharmacies, outlets and establishments to upload data on current drug
prices and inventories. This supports Republic Act 9502 Universally Accessible
Cheaper and Quality Medicines Act of 2008. The major output of this system is a
Drug Price Report where the lowest and highest prices per drug can be

Test
Operations
and
Management
Information
System: This is an integrated system for an efficient and effective accreditation of
drug testing laboratories and related facilities; operations, monitoring and quality
assurance drug test operations; integration with rehabilitation centers; sharing and
retrieval of relevant information for decision-making and policy formulation of
various agencies using drug testing data. Major outputs are: Report on the number
of clients with positive and negative results; Number of accredited drug testing
laboratories and rehabilitation centers; Number of clients with confirmed positive

Adverse Drug Online Reporting System: Drugs may have side-effects and in
some cases can cause adverse reactions. It can cause grievous damage to the
users or even result to death. The Adverse Drug Online Reporting System enables
the hospitals and drug companies to report cases of adverse reaction(s) on the use
of drugs. It can help the DOH and drug companies to track and manage information
on adverse drug reaction; manage drug risks and address safety concerns of

Health Record Number Registration System: A medical record, health record,
or medical chart is a systematic documentation of a patients individual medical
history and care. Through the Health Record Number Registration System, the
patient is assigned a unique identification code or number on his first contact with
the health facility hospital (admission, emergency room or outpatient clinic) or
private clinic. This system facilitates integration of data by linking health records
that are physically located in different places. [Architecture Online; Current

Health Facility Database System: Health facilities are places that provide health
care. These may include hospitals, clinics, outpatient care department and
specialized care centers, e.g. birthing home and psychiatric care centers. For
effective health information systems, a complete and reliable inventory or database
of health facilities is critical. The Health Facility Database System establishes the

DOH central database that geographically references all health care facilities in the
country. All licensed health facilities approved by the Bureau of Health Facilities and
Services, data submitted by rural health units and barangay health stations were
provided with unique codes. To expand the usefulness of the database, there is a
provision to enter the different services provided by a health facility, facilities and

Hospital Reporting System: One method to relay health information is through a
standard or uniform collection of data on morbidity, mortality, and other required
conditions. The Hospital Reporting System enables electronic sending of data from
hospitals on a daily, monthly, quarterly and annual basis. The data sets conform to
the requirements of the National Center for Health Facility Development and Bureau
of Health Facilities and Services. [Architecture Online; Current Status: Being

Integrated DOH Licensing Information System: Harmonization and streamlining of
systems and processes for licensing, accreditation and certification to make health
regulation more rational and client-responsive. [Architecture Online; Current
Status: Ongoing development of the system design solution].
Field Health Services Information System: An online uploading system where
data from health facilities can be electronically submitted to the DOH. Data on
health services given to patients (e.g. immunizations, family planning, maternal
care, and others) can already be submitted even on a daily basis. The major
outputs are the Field Health Services Reports. [Architecture Online; Current Status:
Ongoing development of the system design solution].
Health Clinic Information System: An information system designed for rural
health units and barangay health stations. It captures patients general information,
health services done (immunizations, maternal care, family planning, others),
laboratory examinations and diagnosis. Further, it can extract FHSIS data for
uploading into the DOH. [Architecture Client Server; Current Status: Ongoing
Testing].
Schistosomiasis Information System: An information system designed for
reporting schistosomiasis cases from health facilities to the DOH. [Architecture
Hybrid of Offline & Online; Current Status: Operational].
Filariasis Information System: An information system designed for reporting
filariasis cases from health facilities to the DOH. [Architecture Hybrid of Offline &
Online; Current Status: Operational].
Integrated Chronic Non-Communicable Registry System: This is an online
reporting of cancer, diabetes, COPD, and stroke data from health facilities. It also
includes the uploading of data from the Philippine Renal Disease Registry System.
One important feature of this system is the centralized issuance of patient
identification code or number for better analysis of the risk factors and program

interventions to be done by the DOH. [Architecture Online; Current Status:

Overseas Foreign Worker Clinic Uploading System: An online uploading
system where data (results of the laboratory and psychological assessment FIT or
UNFIT to work) from accredited OFW clinics can be electronically submitted to the
DOH. [Architecture Online; Current Status: Ongoing Testing].
Philippine Organ Donor and Recipient Registry System: An online system of
collecting data from kidney organ donors and transplant candidates, and automatic
matching of donors to kidney transplant candidates. The system supports the policy
on organ donation and transplantation as well as the integration of donor and
transplant candidate data from different sources. [Architecture Online; Current

Philippine Persons with Disability Registry System: An online system of
registering persons with disabilities. This system was developed to support the
National Council on Disability Affair. [Architecture Online; Current Status:
Operational].
Document Tracking System: A tool for managing/tracking document flow and
monitoring the status from the time the document is sent out from an office up to
such time that the document has been acted upon. [Architecture Online; Current
Status: Developed and for Implementation]
Electronic Job Posting: A centralized venue for electronic posting of job vacancies
in the DOH, other government agencies, local government units, and private
facilities/organizations involved in health care. [Architecture Online; Current
Status: Operational]
Personnel Information System: A system of recording data on personnel
(personal information, education, training, services or employment, and eligibility
data), updating of profiles, management and monitoring, and generation of service
records and other statistical reports. [Architecture Client Server; Current Status:

Payroll System: A system to facilitate and simplify the monthly and annual
preparation of general payrolls, reports to the Bureau of Internal Revenue,
Government Service Insurance System, Pagibig, and other requesting

Information System Database: A database of all approved information system
projects submitted to Information Management Service for clearance. [Architecture

National Database on Health Human Resource: A database of all health related
manpower in the country. [Architecture Online; Current Status: Operational]
