QG PROCESS SAFETY
One important issue in maintaining a good
Process Safety Culture in any
organisation is to remember the past and
learn the lessons from the previous
incidents. We must always remember and
respect the hazards associated with our
business process.

QG - 2010 INJURY ANALYSIS


2010 was the first time we separately counted
Accidents and Near Misses from all the Incident
Notification Reports received. Accidents were
Analysed various aspects such as, location,
activity, causes, injuries, fires. Moderate to high
potential accidents were specifically looked into.
Following are the highlights:

What Can You Do?

Be vigilant about the hazards of the


materials and processes in your
plant.

Recognise near miss events to


remind you of what could have gone
wrong.

Use incidents which occur in other


facilities, such as the incidents
reported in the Oil & Gas Industries,
to remind you of the possibility of
similar problems at your plant.

Always operate within safe operating


limits, and established operating
procedures. When this isnt possible,
notify your supervisor immediately.
Use approved methods for
authorising changes to established
procedures, including a thorough risk
evaluation and approval by
knowledgeable authorities.

Safety, Health, Environment & Quality performance. This
means the protection of the health and safety of our people
and of those affected by our business, the protection of our
physical assets and reputation and the protection of the
environment.

We all at Qatargas recognise that SHE&Q is everyone's
responsibility and that we each have a duty to intervene to
prevent unsafe actions and to reinforce good behaviors.

No Lost Time Injuries


No major fires
Six Restricted work cases and 13 cases of
Medical Treatment
Out of 48 injuries and illnesses, 34
happened to either hand or leg (about 71%)
29% of cases were found to be in slip/trip/fall
category followed by struck by (17%) and
caught between objects (15%)
Out of 11 minor fires, eight took place while
the equipment was in operation and two
involved waste
72% of all accidents took place while
equipment was in operation.
Among the medium to high potential
accidents, 58% of them took place in QG-1
and Offshore (29% each).
When looked at asset wise distribution of all
accidents, it was found that 23% of
accidents took place in QG-1 and 33% at
Offshore.

In 2011, for the first time Qatargas OPCO will enter a major
challenge to complete 4 planned maintenance shutdowns in one
year.

To achieve this milestone, this challenge requires the


involvement and extra effort by all departments.
It will require commitment towards the highest safety standards
to complete the planned maintenance shutdown activities safely
and successfully without any lost time injuries.

MUST FOLLOW
In 2010, Management set the Qatargas 10 golden behaviors of conformance.
These points MUST become the culture, not just a piece of paper !!
1. Commit yourself to the Qatargas Direction statement, vision and objectives.
2. Behave in a safe manner, seek to stop others from committing unsafe acts.
3. Read, understand and comply with Policies and Procedures. Complete all
Mandatory training.
4. Initiate the MOC process for gaps that you identify.
5. Lead others by example of your good conformance behaviors.
6. Come to meetings on time and be prepared. Show respect to others.
7. Take responsibility for your actions.
8. Deliver against deadlines and commitments.
9. Understand your internal and external customer requirements. Meet or exceed
them.
10. Speak up. Do NOT tolerate non conforming behavior in others
We will go beyond compliance with local regulation to meet or exceed
internationally accepted good practice in health, safety and environmental
management.
