5 Choices Benchmark Instruction

What is a 5 Choices Benchmark?


5 Choices Benchmark is an assessment to measure your productivity to show where you stand on key productivity behaviors base on The
Time MatrixTM. The assessment consists of 29 questions, which gathering feedbacks from yourself, your supervisor, and others (coworkers/colleagues). The result will be most accurate if the assessors are those who work with you closely and are ready to give their opinions
candidly.
Instructions
1. Go to https://assessments.franklincovey.com and choose the
language for your assessment.
2. Registration by click the Create Your Account button. Please
fill in your personal information, create and confirm your
password. (You must select password by yourself which
contain at least 6 alphabets) If you or your assessors are
going to answer the questionnaire in Thai, please select
Thai*** in the Report Language Preference and tick I
accept before clicking Create Your Account

3. Fill in Workshop Registration Code that you received from


PacRim via email by copy and paste it in Workshop
Registration Code box and click Register.

4. Start the assessment by clicking Get Started on your
selected workshop. Please read and follow the instructions on
the page.

5. Send Invitation Email to your assessors by click Email Text
button, copy all the texts in pop-up window and paste it into
your email. Send it to your assessors as follows; (You can invite
your assessors up to 30 people)

6. Take self survey by click Take Self Survey button. When you
completed the assessment, you will see a Thank You
message. The completed assessment cannot be modified.
Change the language of Self-Assessment (See Remark 10)
7. Monitor number of Assessors that how many people have filled
out the Questionnaire for you in Respondents column. Only
categories that meet the minimum, and have changed to
green***, will be included in your report.
***After you click Print Report, the system will reset the
number of assessors and change to red according to the
Privacy Policy.

When your assessors click this link, the


assessments window will pop up.

The numbers of assessors do not match the number of


invited parties. (See Remark 11)
The assessors chose the wrong role. (See Remark 12)
How to check that all the assessors have completed the
assessment for you. (See Remark 13)
8. Print Report. You can print the report on the day you received
a notification email from PacRim*** by clicking View & Print
Benchmark Report. Please bring the printed report to the
workshop. (The report is a PDF file.)

***Prior the available date mentioned,


you will be able to print your report if
you take a "SELF" survey or if your
"Boss" submits a survey. Survey
responses from Direct Reports and
Peers will not be included in the report
until AFTER the date above has past.

The number of minimum increases and the status changed


to red. (See Remark 14)
Comments of assessors in the report are in symbols and
unreadable. (See Remark 15)

You can reset your password by click Forgot Password and follow
these steps;

1) Click on the "Forgot Password" button next to Login button.

4) Click the link in the email. You will find a new window pops up.
5) Type in your new password at least 6 alphabets and confirm

password, then click Submit. You will be able to use the new

Remark 4:
Cannot register Workshop Registration Code. (from step 3)
1) Please Copy workshop registration code instead of retyping it yourself to prevent the typo in similar characters such as O and 0.
2) Ensure that before and after the code does not have space. Please remove all spaces before and after the workshop registration code.

1) You can change the language of Email Text and website at the main page before login to the system.
2) If you have already login to the system, you have to logout and change the language at the main page. Then you can login to the
system again.

Remark 6:

If the email message only shows its subject without body text and link to the assessment, please reopen the Email Text pop-up by right
clicking the Email Text button and choose open in new tab. Then, copy all the text and paste in your email manually and send to your
assessors.

Remark 7:

assessment has been completed. Please note that the
completed assessment cannot be modified.

Please ask your assessors if they completed the assessment, if they chose the right role and whether they saw a thank you message. If the
thank you message does not show, the assessment will not be recorded. Please ask the assessors to complete the assessment again.

If your assessors chose the wrong role, you or the assessors must provide PacRim the following information:
1) What is the role that was chosen incorrectly and which role to switch to?
2) What was the date and approximate time of that your assessor completed the assessment?
3) Did your assessor answer questions no. 2 3? If yes, what are the answers?

Remark 13:
How to check that all the assessors have completed the assessment for you. (from step 7)
Since the system does not store personal information of the assessors due to the privacy policy, PacRim cannot check if your participants
have already completed the assessment or not. Please kindly ask your invited assessors if they completed the assessment.

Remark 14:
The number of minimum increases and the status changed to red. (from step 8)
After you printed out the report, the minimum number will be automatically recounted and will add 3 more people to each role to reach the
minimum number and the boxes will turn red due to privacy policy that requires at least 3 assessors for each round.
1) If there are no more assessors to do the assessment, you do not have to send the invitation email to reach the minimum number of
assessor again. You can print out the report as many times as you like. The re-print report will show the same data.
2) If there are more assessors that would like to do the assessment for you after you have already print-out the report, the new report will
not complete and cannot print-out unless the minimum number of assessors are reached again (the box that shows number of
assessor turns to green)

Remark 15:
Comments of assessors in the report are in symbols and unreadable. (from step 8)
Due to English report cannot display Thai language; the comments from assessors are displayed as unreadable symbols. You can fix this
problem by yourself. You can reprint the report with the same data, if there is no additional assessor. The report can be reprinted as many

You can change the language of your report by going back to the home page of this website, click Edit Profile and choose Report

If you registered with the Workshop Registration Code instead of your password, you will be unable to login the online assessment system.
