Preparation

Ensure everyone is trained

and knows the task

Ensure relevant personnel

are aware of the tasks to
be conducted

Assess environmental
conditions ( weather, wet
ground etc.)

Ensure the right tools and

equipment are at hand

Confirm initial and final

location of load and lift
routes with relevant
personnel

Ensure final location of

load is free from
obstructions

Ensure JSA for lifting ops is

revised/ reviewed prior to
ops

Supervised operations one person in charge (PSS) & team workminimum 3 competent personnel in a lifting team (crane operator;
banksman, rigger/ slinger)
Personnel training and certification: crane operator; banksmen, riggers/
slingers (PM)
Planning of activity: Lift Plan (PSS)
o
Assess the load- characteristics and restrictions (PSS)
o
Specific Equipment layout ensure lifting routes are clear of
obstructions; mark / highlight all proximity hazards ensure
final position for load is adequate (PSS)
o
Suitable equipment available (adequate SWL, certification
and color code) (PSS)
o
Certified Equipment- crane & lifting appliances and
accessories (PSS)
Everyone aware of the task TBT and JSA, LMRA- as applicable.
(PSS)
Ensure personnel is fit for work (physically, psychically) (PSS)
Ensure that personnel not involved in operations is kept clear of
working area no persons should be positioned under a suspended
load or between suspended loads and fixed objects (PSS)
Define roles of personnel and establish positioning during operations
(VC&PSS):
PSS to ensure communication with relevant personnel (PSS)
PSS to obtain confirmation from vessel and crane operator that the task
can be safely conducted (PSS)
Operator to visually check equipment before task in good condition;
clear of slippery substances- oil, mud etc.;
Ensure safety devices are maintained as operational and that operators
know the location of these devices and how to use them (VC)
Protective equipment to be worn as per the above (operator)
Conduct trial lifts if in any doubt (PSS)
Consideration given to environmental conditions prior to starting work
No work to be undertaken if weather conditions (wind, rain etc.) affect
the safety of the lifting operations (PSS & crane operator).

Masuri de control / Control measures

Ground surfaces must be inspected to ensure sufficient compaction to
operate on, if in doubt seek advice from the Site Manager (crane
operator & PSS)
Plan the travel path and ensure route is clear of obstructions and
maintain a safe distance from all site workers (PSS).
Check for and ensure no overhead power lines are within the crane
radius (within 3 m) (PSS & crane operator).
Additional control measures are required if the work is within the above
distances and the task is NOT to continue until adequate controls are in
place. (PSS)
Before operating the Crane check for and ensure all ground workers
and other vehicles are well clear of the work area (PSS & crane
operator)
Ensure reverse beepers are audible and flashing lights are working at
all times while moving (crane operator)
Operating speed when driving the Crane must be done at creeping or
extremely slow speed (walking speed) (crane operator)
Ground surfaces must be inspected to ensure sufficient compaction to
operate on, if in doubt seek advice from the Site Manager (crane
operator & PSS)
No work to be undertaken if weather conditions (wind, rain etc.) affect
the safety of the lifting operations (PSS & crane operator).
Ensure that personnel not involved in operations is kept clear of
working area (VC)
Maintain good communications (operator)
Team work; lifting/lowering only under supervision; the assistant to
ensure lifting/ lowering equipment is safe- give clearance for lowering
equipment
Load attachments to crane to be completed by qualified riggers (PSS).
Choose a sling of adequate capacity and determine the best means of
attachment (PSS & rigger/ slinger)
Position the lifting assembly over the centre of gravity and aim for a
direct lift (rigger/ slinger)
Ensure all workers, including the riggers are well clear before lifting the
load (PSS)
Smoothly and gradually raise the load to max. 1 m off the ground and
hold. Check the slinging and balance of the load (crane operator &
banksman).
When the load is stable, continue the lift until it is high enough to clear
all objects in its path (crane operator & banksman)
Tag Lines are to be used if the load is raised above shoulder height
(operator)
Maintain visual sight of crane driver at all times; If crane driver is out of
sight, lifting is to stop immediately (banksman)
Ensure safety devices are maintained as operational and that operators
know the location of these devices and how to use them.
Protective equipment to be worn as per the above- incl. safety harness
(operator)
