CHAPTER I
INTRODUCTION
The power of a healing environment comes from the design details that
empower patients to take responsibility for their own health. 1
The role of a medical institution, a general hospital perhaps is to meet their
patients needs and provide treatment for whatever disease they may have. The
facility can nurture their patients until they are fully recuperated. After the
medication process the patient can easily be discharged and be taken care of at
home. However, in medical facility dedicated for treating psychiatric patients, this
is not always the case.
Society, through time, may have developed a negative perception about
patients who have been discharged from a psychiatric or drug rehabilitation facility.
Unlike patients released from a general hospital where they can be taken care of
at home and continue life once they have fully recovered, post psychiatric and post
drug rehab patients must have a continuous professional nursing to avoid further
impairment or in worst case recurrence of the disorder.
Mental home care facilities have undergone evolution and is greatly
influenced by the colonizers of the land. Psychological handling evolved from the
form of treatment introduced by the Spaniards at Hospicio de San Jose, to the
scientific approach by the Americans. Later on National Psychopathic Hospital in
Mandaluyong was established to meet the demand of increasing number of mental
disorder cases in the metro. Due to this occurrence the Department of Health had
given priority to programs and projects that will address the needs of those who
are mentally ill. 2
Various private and public institutions dedicated in providing professional
home care in the country are available. One of the most common facilities are
those dedicated to the old ones. Custodial care are given to those patients until
they reach the twilight of their existence. Existing facilities for post psychiatric and
post drug rehabilitation patients are also available but are not very prominent.
H.I.V.E. which stands for Harmonious and Integrated Viable Environment,
is a private home care sanctuary dedicated in providing professional care and
nurture for post psychiatric and post drug rehabilitation patients. The facility is

situated on a private lot at Cararayan, Naga City with a rural setup. The country
environment provides comfort thus giving its patients relief. The sanctuary is also
accessible to different medical facilities in cases of emergency.
The proposed complex will be a pioneer in accommodating patients from
mental and drug rehabilitation facilities in the entire region. Since the institution is
a medical facility which will require necessary number of professional nurses and
caregivers, sourcing out will not be a problem because these professionals are
highly obtainable in the area. The locations ease of access is also of great
advantage to families and loved ones who are willing to visit and monitor the
progress of their patients.

I.

Background of the Study


Home care facility or also known as nursing home facility is a medical

institution that delivers a high level of care for older adults, mentally disabled, and
other folks who needs continual nursing care due to difficulty in coping up with daily
activities. Nursing homes offer a so called custodial care, high level of medical
care, and various types of rehabilitative therapies to support the facilitys patients.
Resident physicians, nurses and other medical professionals are always on the
premises supervising.3 Skilled nursing care is available for 24 hours allowing the
delivery of medical procedures and therapies on the site manageable. This
services though cannot be found in other housing institutions
The advent of home care facilities started in the late eighteenth century
when womens and church groups had seen the need to cater special nursing
services for aged and incapacitated persons. Such institutions have yet existed not
until 1850s when Bostons Home for the Aged Women was established.4 Although
the special home was established to support elderlies without substantial familial
support, they still require entrance fee. Most people in the early 90s hardly looked
upon long term home cares as a solution to the demands of the elderlies. Instead
the institution had become a symbol of deprivation and abandonment. By the
1930s however, when people had seen the essential role and function of the
nursing homes, government officials accepted that the rising portion of the elderly
people is in necessity of such facility. Investigations to the industry in the 1970s
had shown substandard care forcing lawmakers to provide regulations to ensure
a quality professional nursing care.
Psychiatric wards or mental hospitals are sub-units of a general hospitals
specializing treatments for serious mental disorders such as clinical depression,
schizophrenia and bipolar syndrome. A specialized and controlled environment is
required as part of routine assistance of inpatients. Treatment as to long-term or
short-term may vary depending on the risk factor of a patient. Same thing of a drug
rehabilitation facility, doctors administer medications and assistance to control
mental and physical drug abuse triggers. Patients in a drug rehabilitation program
are trained to end their so called toxic relationship or those that prompts them to
drug abuse. Families and loved ones are also included in ones rehabilitation
through counseling. Therapies usually last for 28 days but may vary depending on

patients healing progress. After therapeutic procedures in both facilities,


medication may be continued at home or in a home care facility.
Philippine government, at present, has laws regarding the governance of
the provision of mental health services and finances around 3% of the total health
budget on operation and maintenance of mental health facilities. 5 There has
already been existing long-term healthcare for post psychiatric and post drug
rehabilitation patients in the Philippines but people are somewhat unaware. There
are numerous number of nursing homes of the same type in the country but not as
abundant compared in other countries.
The sanctuary as a project aims to provide accommodation with long term
professional care using country and inviting atmosphere from patients in mental
and drug rehabilitation facilities in the entire Bicol region.

II.

Statement of the problem


A psychiatric institution same with a drug rehabilitation facility provides

medication for only a limited timeframe. As per policy by the only mental institution
in Bicol Region which is the Don Suzano J. Rodriguez Memorial Mental Hospital
of the Bicol Medical Center, there is only a maximum of three months treatment
for psychiatric in-patients. For drug rehabilitation centers a maximum of nine
months treatment is required.
After treatment, patients are then being discharged and returned to their
families. Dra. Lalyn Marzan, a resident forensic psychiatrist of the mentioned
institution, said that acceptance of the patients inflicted with mental illness has
always been the biggest issue with their families. Relatives seldom welcome their
patients warmly leading to emotional deprivation, feeling of rejection, and in worst
case a relapse of mental ailment. Hence a home care facility specifically for post
psychiatric and post drug rehabilitation patients is essential.
Although acceptance has been a major concern for families due to
uncontrolled conditions of their patients, they still seek for a place for them to be
located. A place where patients will be assured of their security and full time
custodial care. Through this patients will be given sustenance by their families and
make their patients feel that they havent been neglected.

According to the 2010 figures of the National Statistics Office, mental illness
is the third most common form of disability, next to hearing and visual impairment
in the Philippines. The most reported case is due to family hereditary and the abuse
of narcotic drugs and other chemicals that may lead to mental disorder. In Bicol
region there is an average of about 6,300 reported mental illness case and 2,745
drug rehabilitation patients in the entire country. Yet, there is no existing facility
that caters a professional nursing care needed by these patients after medication
in the entire region. The nearest facility that would meet the needs of post
psychiatric and post drug rehab patients can still be found at the nearby region,
the Lourdes Homecare Medical Clinic at Dasmarias Cavite and Estrellas Home
Care Clinic & Hospital, Inc. along Quezon City. The mentioned facilities are too
distant that it would be impractical to house patients from Bicol region.
There is a discrepancy between medical professionals and places for them
to work at. Experts in medical field often work in agencies not in line with their
profession where their skills are occasionally used or not used at all. The lack of
facility on the other hand forces patients who are in need of a home care to admit
themselves outside the region. Thus depriving patients and their relatives the ease
of mobility and accessibility to a correlated institution.

III.

Project Objectives
H.I.V.E. (Harmonious and Integrated Viable Environment) Homecare

Sanctuary intends to provide professional care to post psychiatric and post drug
rehabilitation patients. To come up with an efficient home care the project aimed
for the following objectives:
1. Create an available facility for patients who have been deprived with
acceptance by their families and loved ones.

2. A home care that will offer services for the entire Bicol region through a rural
setup and is accessible to other medical facilities in case of emergency.
3. An institution suited for the programs of the users and will be joining natures
assets with modern architecture.

4. Create a facility that would offer job opportunities to medical professionals.

IV.

Significance of the Study


The H.I.V.E. (Harmonious and Integrated Viable Environment) Home Care

Sanctuary development will be of great importance to the following:


A. To the post psychiatric and post drug rehabilitation patients

The facility will be the first in the entire region to cater services
specifically for the mentioned type of patients. The project means a
suitable place for them with enough facilities and activities to cope
up with their daily mind settling and treatment. A 24 hour professional
program care will run as needed by post psychiatric and post drug
rehab patients.

B. To medical professionals and creative workers

The study seeks to offer job opportunities for medical professionals


to supervise the patients. The sanctuary would also support jobless
creative workers skillful enough on the field of livelihood and
enterprise. This would be essential and incorporated in the patients
activities as part of their therapy.

C. To the region

As a pioneer for catering home care services in whole Bicol region,


the development provides reasonable ease of access to the
institution from the people of the area. The project is of great
importance in delivering availability of a facility especially for post
psychiatric and post drug rehabilitation patients. This will include the
accessibility through different medical facilities in Naga city.

D. To the future researchers

The project is also of great significance to future researchers who will


be studying the same type of institution. Reliable data will be
provided as part of feasibility of the research. The book can be a
reference for medical enthusiast educating themselves in line with
the field.

V.

Scope and Delimitations


The research is mainly focused on the development of the H.I.V.E.

(Harmonious and Integrated Viable Environment) Homecare Sanctuary. The study


is limited to post psychiatric and post drug rehabilitation type of patients in the Bicol
region. Research process does not include the study of various mental illness as
well as medical and surgical treatments of the mentioned type of patients. The
project does not also cover the study of marketing type and related livelihood
programs. A holistic and modern scheme to cope up with the needs and
necessities of the users will be applied to the project.
The project will be not be including associated engineering studies and
drawings. Details and structural studies and analysis will also be excluded from
the research. Thus, the research will be limited to the design process, floor plans,
elevations, sections, perspectives, some details and material specifications of the
following spaces and areas in relation to its proposed environment.
A. Administrative facilities
These shall be provided for business transactions, admissions,
social services, medical and financial records, and administrative and
professional staff. Provisions for private interviews shall be included.6

B. Public facilities

The type and size of the public facilities depend to some extent on
the type of sponsorship of the LTC facility; but one factor is constant: the
number of visitors in the LTC facility is much smaller per patient than in an
acute general hospital. This is often reflected in parking criteria and internal
visiting areas. Where the general hospital may require one visitor parking
space per bed, the LTC facility requires one visitor parking space for
between 3 and 20 beds. Architectural features that are desirable. When the
building is large enough, a small lobby with perhaps a snack and gift shop.

may be provided for it, but these would be similar to what is provided in a
community supported general hospital.7

C. Medical Facility

Legislative requirements and medical practice require that major


treatment of the acutely ill patient in the LTC be available within general
medical and surgical hospitals. This gives the community an economical
use of both staffing and facilities. Sometimes chronic hospitals in nonurban
areas provide as part of their facilities intensive medical and surgical units;
but with the notion of regional health care, this is not considered by most
health planning agencies to be desirable today. The facilities in the LTC
which are provided, where the law permits, are a diagnostic x-ray unit, a
laboratory for hematology, biochemistry, etc., and, usually as part of the
nursing unit, treatment rooms . It is desirable that spaces for dentistry,
podiatry, and, on occasion, optometry, be provided if the patient population
can support them.8

D. Dietary service
In the LTC as in the general hospital, the dietary requirements and
the space and equipment required to support them are extensive and the
basis for involved research and analysis.9
E. Storage areas

The elements of hospital storage should be provided, and the


importance of ensuring the flexibility of the compartments for this cannot be
overemphasized.10

F. Work area and maintenance area

In general these are quite similar to those of the community hospitals,


except that there is a minimum of medical equipment to maintain and that,
although the number of patients may be similar to the general hospital, the
total amount of equipment in the LTC requiring maintenance or repair is

considerably less. We have found a single large room to be more than


adequate in these areas for most LTC facilities.11

G. Mechanical facilities
During the last few years, the LTC facility has been designed to meet
the structural and mechanical standards of the general hospital. As in most
other medical facilities, problems, particularly of fire safety, have required
fireproof buildings, often with sprinkler protection, smoke detectors, zoned
floor areas, and rigid standards of fire resistance in terms of flooring,
surfaces, and materials used. It is, of course, a fact that fire safety in a
building housing many patients with behavioral problems is one of the
major, if not the major factor in construction.12

H. Nursing units and Supportive Facilities


Nursing units are groups of resident rooms, staff support areas,
service areas, and resident support areas whose size and configuration are
based upon organization patterns of staffing, functional operations and
communications as provided in the functional program for the facility.13

Criteria for Site Selection


Bicol region is the largest island in

Philippine archipelago that has a total land area


of 18,054.3 km2 (6,970.8 sq. mi), or 5.9% of the
total land area Of the state. The region has also
been well-known for its famous Mayon Volcano.
The area is also rich in agricultural lands making
50% of residents livelihood depend on the
industry.
Naga city has been the central of business

and industries in the region. The largest city has


also been the center of trade, tourism, and
commerce with its busy and large terminals. This
made the city independent from the region.
The metro is home to the two largest
hospitals in Bicol region, the Mother Seton
Hospital which is privately owned and Bicol

Medical Center run by the government. Two other


medical facilities where located along with
commercial facilities to cater services from the
city and other towns. The city has also produced
hundreds of doctors specializing on various fields
and topnotch medical professionals.

fact there is no facility that would support after

rehabilitation in the entire region as of now.
In line with this, various analysis as to
where a home care facility can ideally be located
will be conducted. SWOT analysis will determine
both positive and negative attributes of the three
proposed locations. Together with the SWOT is a

The table 1.0 shows the different pros and cons of the three proposed sites.
Positive and negative criteria on the actual site were considered in selecting the
most suitable location. All three sites shared the same positive criteria except that
San Isidro, Cararayan showed a lot more accessibility to other medical facilities
than the rest of the other sites. The chosen location also showed lesser weakness
and threats making the place ideal to the project.

to the criteria needed for the site. Each site were given score in compliance to the

provided site criteria and is based on the current status of the location. San Isidro
Cararayan Naga City got the highest summation of percentage as an ideal site for
a Nursing Home. Both SWOT Analysis and Site Selection & Justification showed
the same results as to where the project be ideally be located among the three
provided locations.

3. Site Description

Nagueos speak English especially during business transactions, Filipino


as the National language and Bikol as the regional dialect. Naga has a
tropical climate. It has three evident seasons: wet and rainy season from
June to October; cool and dry season from November to February; and hot
and dry season from March to June. It is the commercial, economic,
educational, religious and cultural center of the Bicol region.14
Naga City is Bicol Region's economic, commercial, trade, tourism,
medical, and business, technological, religious, cultural, industrial,
educational, financial, and social center. It is also Camarines Sur's most
populous city. It is also the regional center of trade, commerce, and industry,
as it is home to large international companies, high-rise hotels, and
