This survey for middle school students is about health behavior. It has been developed so you can
tell us what you do that may affect your health. The information you give will be used to improve
health education for young people like yourself.
This survey is anonymous and the answers you give will be kept private. No one will know what
you write. Please be truthful and answer the questions based on what you really do.
Completing the survey is voluntary. Whether or not you answer the questions will not affect your
grade in this class. If you are not comfortable answering a question, just leave it blank.
The questions that ask about your background will be used only to describe the types of students
completing this survey. The information will not be used to find out your name. Again, no names
will ever be reported.

3. SAD OR SUICIDAL FEELINGS

The next 6 questions ask about sad feelings and attempted suicide. Sometimes people feel so
depressed about the future that they may consider attempting suicide, that is, taking some action
to end their own life.
Why do we ask?
Prevalence of Child and Adolescent Mental Disorders
Four million children and adolescents in this country suffer from a serious mental disorder that
causes significant functional impairments at home, at school and with peers. Of children ages 9 to
17, 21 percent have a diagnosable mental or addictive disorder that causes at least minimal
impairment.
Half of all lifetime cases of mental disorders begin by age 14. Despite effective treatments, there are
long delays, sometimes decades, between the first onset of symptoms and when people seek and
receive treatment. An untreated mental disorder can lead to a more severe, more difficult to treat
illness and to the development of co-occurring mental illnesses.
In any given year, only 20 percent of children with mental disorders are identified and receive
mental health services.
Consequences of Untreated Mental Disorders in Children and Adolescents
Suicide
Suicide is the third leading cause of death in youth ages 15 to 24. More teenagers and young adults
die from suicide than from cancer, heart disease, AIDS, birth defects, stroke, pneumonia, influenza
and chronic lung disease combined.5 Over 90 percent of children and adolescents who commit
suicide have a mental disorder.
In the United States in the year 2002, almost 4,300 young people ages 10 to 24 died by suicide.
School Failure
Approximately 50% of students age 14 and older who are living with a mental illness drop out of
high school. This is the highest dropout rate of any disability group.
How results impact instruction?
Data is used to exam at risk behaviors and helps to drive student interventions and health
curriculum.

How data impacts instruction?
Understanding how students perceive the dangers of a drug allows us to better address
instructional strategies in the health classroom.
Data is used in delivery of health curriculum to help students realize that most of their peers have
negative attitudes about using drugs and alcohol and that they are in the majority by making
healthy decisions.

5. TOBACCO USE - SMOKING & SMOKELESS TOBACCO

The next 10 questions ask about tobacco use.


Why do we ask?
These questions are designed to determine the percentage of students that self-report use of
nicotine products.
Teens that smoke cigarettes are 14 times more likely to use marijuana.
Most smokers start during their teen year and teens are the most likely to become addicted.
If a person doesnt start smoking before the age of 19, it is unlikely they will ever start.
How data impacts instruction?
Understanding use of nicotine helps us to create additional interventions related to smoking.
Based on data from last year, it is easy to make statements like 99% of Gates students last year
were not current users of cigarettes. This demonstrates that healthy decision making around
smoking is the norm for middle school students and not smoking in middle school makes you part
of the strong majority that makes healthy decisions.

7. ALCOHOL USE

and liquor such as rum, gin, vodka, or whiskey. For these questions, drinking alcohol does not
include drinking a few sips of wine for religious purposes.
Why do we ask?
These questions are designed to determine the percentage of students that self-report use of
alcohol.
5,000 young people under the age of 21 die as a result of underage drinking every year.
People who reported starting to drink before the age of 15 were four times more likely to also
report meeting the criteria for alcohol addiction compared to those that waited until they were at
least 20 years of age.
More than 67 percent of young people who start drinking before the age of 15 will try an illegal
drug.
Alcohol use is also a strong predictor of other risk factors.
How data impacts instruction?
Understanding use of alcohol helps us to create additional interventions related to alcohol.
Based on data from last year, it is easy to make statements like 97% of Gates students last year are
not current users of alcohol. This demonstrates that healthy decision making around alcohol use
is the norm for middle school students at Gates. Gates students would be in a small minority by
using alcohol.

8. MARIJUANA USE

Why do we ask?
These questions are designed to determine the percentage of students that self-report use of
marijuana.
Teens that use marijuana are 2 times more likely to suffer from depression.
Teenagers 12 to 17 who smoke marijuana weekly are three times more likely to have thoughts of
committing suicide.
Daily use of marijuana among girls is associated with a fivefold increase in the odds of developing
depression and anxiety.
How data impacts instruction?
Understanding use of marijuana helps us to create additional interventions if necessary related to
marijuana use.
Based on data from last year, it is easy to make statements like 97% of Gates students last year are
not current users of marijuana and most students have never tried marijuana. This demonstrates
that healthy decision making around marijuana use is the norm for middle school students at
Gates. Gates students would be in a small minority by using marijuana.

Why do we ask?
These questions are designed to determine the percentage of students that self-report that they are
sexually active.
More than 1 in 4 teens who gave birth were between the ages of 15 and 17.
Pregnancy and birth are significant contributors to high school drop out rates among girls
The children of teenage mothers are more likely to have lower school achievement and drop out of
high school, have more health problems, be incarcerated at some time during adolescence, give
birth as a teenager, and face unemployment as a young adult.
How data impacts instruction?
Understanding the number of students that are sexually active helps us to create additional
interventions if necessary related to sexual activity and other high risk behaviors.
Based on data from last year, it is easy to make statements like 96% of Gates students last year had
never had sex. This demonstrates that healthy decision making around practicing abstinence is the
norm for middle school students at Gates. Gates students would be in a small minority by being
sexually active during middle school and the pressure is to abstain from sex.
