Self-Assessment Form

The performance evaluation process is a positive management development tool used to assist in
career progression. In preparing for your performance review, please complete and return this selfassessment form to your Supervisor. The purpose of this tool is to promote a proactive and
collaborative performance review. Your time and efforts are sincerely appreciated.

2. What are your accomplishments?


a. Establishing Project Quality Management system to the scope of
works.
b. During the recent Operations Audit, Appreciation from the Quality
Director that ICC Project PQMS will be considered as role model
PQMS and the same will be implemented on upcoming projects in
India.
3. What items can you improve?
I believe I can always get better, and would like an opportunity to gain
new experiences in other sector projects that will help me become better
at anything I undertake.

4. What aspects of your position give you the greatest satisfaction at Hill?
Being a key member of the Team in QA/QC perspective.

5. What aspects of your position give you the least satisfaction at Hill?

Sometimes I feel as one man army in Quality on this project. I feel the
project should engage more competent staff to achieve the objectives
of the project as a team.

6. Can you identify any personal and organizational obstacles that stop from you achieving your
objectives? (Organisation - consider company / department / policy or procedures). Have you
any idea how they could be eliminated?

I believe the organization should continue to Encourage, motivate,


reward and recognize the people.

7. To increase your effectiveness, what additional things might be done by your supervisor, you,
the company?
Supervisor: Additional workspace may be required.

8. What specific training/development would be useful to improve your performance or your


career prospects that contributes to the improvement of the business?
