The performance evaluation process is a positive management development tool used to assist in
career progression. In preparing for your performance review, please complete and return this selfassessment form to your Supervisor. The purpose of this tool is to promote a proactive and
collaborative performance review. Your time and efforts are sincerely appreciated.

Sometimes I feel as one man army in Quality on this project. I feel the
project should engage more competent staff to achieve the objectives
of the project as a team.
