RESUMEN
Las recesiones gingivales, son un problema que nos afecta a los ortodoncistas en
la prctica clnica diaria. Numerosos estudios se pronuncian al respecto; sin embargo las
conclusiones a las que llegan son controvertidas y las causas de la aparicin de
recesiones gingivales no se explican con claridad en estos estudios. En la actualidad, las
tendencias no extraccionistas se promueven de forma muy habitual en Ortodoncia. Estas
tcnicas se basan el la colocacin de arcos ligeros de nickel- titanio y colocar los dientes
en la arcada, sin tener en cuenta el permetro de arcada disponible. Somos conscientes
de que ciertas maloclusiones son ms propensas a sufrir recesiones gingivales, aunque
no exista ningn protocolo de actuacin para estos casos.
El objetivo de nuestro estudio es identificar posibles factores de riesgo para el
desarrollo de recesiones gingivales y esclarecer el papel de la ortodoncia en la aparicin
de las mismas. La metodologa del trabajo se ha basado en una revisin bibliogrfica en
Pubmed y ScienceDirect.
Los resultados de los autores revisados son controvertidos. Mientras que en
algunos estudios retrospectivos sostienen que no existe correlacin entre la ortodoncia y
la aparicin de recesiones gingivales, otro tipo de resultados en revisiones sistemticas
estudios retrospectivos apuntan que s existe correlacin entre el tratamiento
ortodncico y el desarrollo o empeoramiento de recesiones. Parece que existe consenso
en cuanto a los factores de riesgo de las recesiones. En la mayora de artculos revisados
se mantiene la idea de que el espesor de enca queratinizada mnimo, el biotipo gingival
y la edad como factores que influyen tanto en la aparicin como en la progresin de las
recesiones. Artculos ms recientes s expresan la problemtica derivada de
sobreexpansionar las arcadas dentarias con la tcnica Damon y similares tendencias anti
extraccionistas, ya que el contenido que suscriben estas tcnicas no se ajustan a la
ciencia basada en la evidencia. Consideramos que ciertos artculos revisados carecen de
rigor cientfico. A la espera de estudios con muestras ms amplias que contemplen todas
las variables que afectan al desarrollo de las recesiones, los resultados que se extraen de
los artculos deben considerarse con cautela.
ABTRACT
Gingival recessions are considered a problem that affects our daily practice as
orthodoncists. Many research are found in the literatura about the etiology of gum
recession, however there seems not to be agreement and the conclusions are
controversial. Thus, the cause why gingival recessions occurs and the exact relation
with orthodontics is still unclear. Nowadays, ther ir a tendency towards non- extraction
techniques, which are based on getting expansion using very light archwires wihtout
taking to account if there is space and bone enough in the arch.
Controversial results are found as regard a possible statistically significant
correlation between the development of gingival recession and incisors proclination..
We are conscious that certain malocclusions are more vulnerable to undergo gingival
recessions, although no action protocol has been described.
The aim of our study is to identify potential risk factors for the development of
gingival recessions in order to clarify the role of the orthodontic techniques. This
systemaic review is based on a literature search on the datebases PubMed and
ScienceDirect.
There is some controversy about the conclusions obtained from the literature
review. While in some retrospective studies is argued that there is no correlation
between the occurrence of gingival recession , other results in systematic reviews
suggest that there exists correlation between orthodontics and gingical recessions. In
most articles reviewed, the authors agree on that the minimum thickness of keratinized
gingiva, gingival biotype and age are factors that influence both the onset and
progression of gingival recessions .In recent studies, the complications associated with
Damon system and similar self- ligating techniquess base don the expansin of the
arches are mentioned. Furthermore, the content that support these techniques is not
suitable to the evidence based science .
Remaining for greater studies, the results that are extracted from articles must be
considered with caution.

Introduccin
1.1 GENERALIDADES
El objetivo de la Ortodoncia es proveer al paciente de una oclusin funcional y
estticamente aceptables con los movimientos apropiados (Gkantidis y cols., 2010). Al
enfrentarnos a un caso, hemos de tener en cuenta mltiples aspectos y en la Ortodoncia,
el periodonto supone un eje fundamental durante el tratamiento. En la mayora de
ocasiones tratar a adultos supone un reto para el ortodoncista, ya que el tratamiento est
dentro de un marco multidisciplinar y de la colaboracin de los distintos especialistas
depender el xito del resultado final.
En los ltimos aos, la demanda de tratamientos de Ortodoncia ha ido
aumentando, incluyendo los casos de adultos, que cada vez son ms frecuentes en la
prctica clnica diaria. Con la mejora de la calidad de vida en la sociedad actual, los
pacientes presentan ms atencin a su aspecto fsico, lo cual produce un aumento de la
demanda de tratamientos ortodncicos en adultos. Estos pacientes padecen
frecuentemente problemas periodontales y evitar el estado inflamatorio del periodonto
es fundamental para conseguir la permanencia en la cavidad bucal de los dientes
afectados. Mediante una ortodoncia de calidad, se pueden aumentar las posibilidades de
supervivencia de dientes con afectacin periodontal severa.
1.2 DEFINICIN
En la literatura encontramos mltiples definiciones para las recesiones
gingivales. En 1972, Lang y Le definen la RG como la exposicin local o generalizada
de la superficie de la raz de los dientes sin acompaamiento de inflamacin ni de
bolsas periodontales. La RG se clasifica como una condicin limtrofe entre la
enfermedad periodontal y un estado saludable (Chatzopoulou, 2014). Warmuz y cols.,
en 2014, explican que la prdida de insercin de tejido conectivo se produce como
resultado del desplazamiento apical del margen gingival por debajo del lmite amelocementario combinado con la exposicin de la superficie de la raz del diente (Figuras 1
y 2).

1.3 PREVALENCIA
Las recesiones gingivales, segn estudios epidemiolgicos, se presentan en entre
un 50% y un 88% en pacientes de 65 aos o ms, y sobre un 50% en sujetos de edad
comprendida entre 18 y 64 aos. Estos datos se traducen en que la prevalencia y la
severidad de las RG aumentan con la edad (Chatzopoulou, 2014). Adems, la frecuencia
con que se desarrollan en pacientes tratados con ortodoncia es similar a la de estudios
epidemiolgicos basados en la poblacin en general (Renkema y cols., 2013). Segn un
estudio de Slutzkey y Levin en 2008, las recesiones gingivales durante el tratamiento
ortodncico se hallaron en un 14,6% de los 303 participantes. La muestra consisti en
23 hombres (18,3%) y 21 (11,9%) de mujeres con presencia de recesiones sin
diferencias estadsticamente significativas entre ambos sexos. Se hallaron recesiones
gingivales en un 14,6% de los pacientes tratados y en un 1,6% del total de dientes
analizados. Los datos de otros autores muestran que las recesiones irreversibles
aparecen tras la aparatologa ortodncica fija entre un1.3% y 10% de los casos tratados
(Djeu y cols., 2002).

Lang y Le, en 1972, publicaron un artculo con el que se sentaron las bases de las
dimensiones gingivales en condiciones de salud y siguen vigentes actualmente.
Concluyeron que mayora de las superficies (>80%) con 2,0 mm o ms enca
queratinizada estaban clnicamente sanos y el 76 % de estas mismas superficies no
presentaban exudado gingival (Figura 4). En todas las superficies dentales con menos de
2,0 mm de enca queratinizada observaron inflamacin clnica y cantidades variables de

BIOTIPO GINGIVAL
Clnicamente, en seres humanos distinguimos dos tipos de biotipos: uno fino y
otro grueso. Un biotipo periodonto fino o festoneado se caracteriza por presentar una
gran discrepancia entre la altura del margen gingival en el punto ms mesial y el
interproximal, tejidos blandos delicados, y un hueso alveolar festoneado. Los dientes en
suelen tener forma triangular. En el biotipo periodontal grueso podemos encontrar
menor discrepancia entre la altura del margen gingival y el rea interproximal, los
tejidos son ms densos y fibrticos y el hueso alveolar es ms plano y grueso. Sabemos
que previamente a la RG, se produce una inflamacin en el tejido gingival. En un
biotipo fino, la proliferacin epitelial ocupa todo el volumen del tejido conjuntivo que
resulta en una rpida recesin de la enca . Por el contrario, con un biotipo grueso, la
inflamacin puede ser contenida y no necesariamente se destruye el tejido gingival
(Chatzopoulou, 2014).
MORFOLOGA SEA
Segn Wennstrm, en 1996, siempre y cuando el diente se mueva dentro del
hueso alveolar, no se producen GR, independientemente de la calidad (volumen) y
cantidad (anchura) de la enca adherida. Como la reabsorcin sea se desarrolla en la
direccin del movimiento, en ocasiones, un espesor mnimo de hueso o apenas
inexistente, es un factor que complica el tratamiento de ortodoncia, producindose
dehiscencias seas (Yagci y cols., 2012). Si el movimiento dental conduce a producir
una dehiscencia sea alveolar, el riesgo de desarrollo recesin es evidente. En
consecuencia, la posicin del diente puede afectar a la morfologa sea alrededor de un
diente. Si un diente se encuentra en una posicin ms vestibulizada en la tabla sea,
existe ms propensin para desarrollar dehiscencias y RG (Chatzopoulou, 2014).
INSERCIN DE FRENILLOS ALTA
Aunque el papel de la insercin de los frenillos en el desarrollo de las RG no est
claro en la literatura, se recomienda a los ortodoncistas examinar con detenimiento la
magnitud de la fuerza de traccin sobre los tejidos gingivales, por el riesgo de que
aparezcan recesiones (Figura 6) (Chatzopoulou, 2014).

MOVIMIENTO ORTODNCICO
Un elevado nmero de estudios en animales han intentado analizar el efecto de la
ortodoncia sobre los tejidos blandos y duros. Parece que los dientes que se mueven
hacia fuera del hueso alveolar, sufren una prdida de hueso y de tejido blando. En
contraste, moviendo los dientes hacia el interior de la cortical se acompaan con un
aumento de hueso (Figura 9) Sin embargo, a pesar de ese aumento de soporte seo,
(hasta en un 50%) no se observ mejora en el tejido blando (Chatzopoulou, 2014). En
una revisin sistemtica de Joss-Vassalli y cols., en 2010, pretendieron esclarecer la
asociacin entre las RG y la ortodoncia; sin embargo, concluyeron que haba una base
de evidencia dbil (con estudios siendo predominantemente de naturaleza retrospectiva
y con nivel de evidencia bajo o moderado) y los resultados de los estudios eran
controvertidos.
Existen movimientos dentales en ortodoncia que consideramos de riesgo. Uno
de ellos es la distalizacin a una zona edntula estrecha, que sucede a menudo cuando
realizamos extraccin de bicspides durante el tratamiento y distalizamos los caninos,
creando un defecto mucogingival si la enca adherida es insuficiente. En el movimiento
de vestibulizacin extremos tambin se adelgaza la cantidad de enca adherida y
suponiendo un riesgo de que aparezca una recesin gingival.

HBITOS AUTOLESIVOS
Los hbitos perniciosos como el hbito de morderse las uas o que contacten con
la enca, as como la presencia de piercings en el labio o en la lengua se asocian a la
aparicin de RG. No es infrecuente encontrarnos con recesiones gingivales en los
adultos jvenes y muchos se relaciona con el tratamiento de ortodoncia pasado y

1.6 CLASIFICACIN
Miller, en 1985, public un sistema de clasificacin de las recesiones gingivales
con notable repercusin, pues en la actualidad el mtodo al que ms se recurre para
clasificar las RG. Este mtodo supone una ventaja para recordar con precisin cada
componente del defecto de la RG y resulta muy til para correlacionar el pronstico de
tratamiento/ resultados y la anatoma de los tejidos. Esta clasificacin fue distinta y ms
completa que las anteriores que existan, ya que esta diferenciacin de las RG permita
el diagnstico, manejo y prediccin del pronstico de las RG tras un recubrimiento
quirrgico. Los conceptos de cada subtipo de la clasificacin de Miller (Figura 12) son
los siguientes:
La Clase I implica que el defecto est localizado a nivel coronal a la unin muco
gingival sin prdida de tejido interproximal (Figura 13: A).
En la Clase II el defecto se extiende ms all de la unin muco- gingival pero sin
prdida de tejido blando interproximal (Figura13: B).
Clase III: es similar a la Clase II pero difiere en que el tejido blando interproximal
perdido est a nivel apical de la unin cemento- esmalte (Figura 13: C)
Clase IV: el tejido blando interproximal de la recesin avanza a hasta un nivel
apical a la parte vestibular que se observa del tejido blando del margen gingival libre
(Figura 13: D).

Case study 1
Female patient M.W., aged 29 and with skeletal class I. In
her childhood, the upper and lower right first premolars had
been removed for orthodontic reasons. The treatment plan
provided for the extraction of the upper and lower left first
premolars, but the patient did not give her consent for this. The
proposed solution entailed intensive approximal reduction of
the enamel on the left side of the dental arches. The treatment
involved a bioprogressive technique using braces with 0.18
slot and segmented arch wires. Figures 4A-C show the
dentition of the female patient prior to treatment on the day
the appliance was removed and one year after retention as
well as after two operations to cover a recession in the area
of the lower left canine. An analysis of head films (Figures
5A and 5B, Table 1) indicated that prior to treatment the APICEJ2-B angle was 19.8 and bone dehiscence height CEJ2-Id
amounted to 0.7 mm. At the end of the treatment the APICEJ2-B angle was 17.3 while CEJ2-Id amounted to 0.9 mm.
The bone parameters in the anterior section of the mandible
deteriorated after treatment. However, they did not exceed the
threshold values. The width of the mentalis prior to treatment
was 13.9 mm, but following treatment it had increased to 15.1
mm. Gingival recession had deepened significantly around
the lower left canine for the value 8 mm and width 7 mm.

GINGIVALES
La aparicin de la recesin gingival, como se hemos definido anteriormente,
supone la exposicin del cemento al migrar el tejido gingival hacia apical y supone unas
secuelas que adolecen el propio paciente que las sufre.
Hipersensibilidad dentinaria, que a veces requieren una restauracin para
solventarla, as como injertos de tejido conectivo y en ocasiones tambin requiere la
realizacin de un tratamiento endodncico.
Frecuentemente la exposicin de cemento perteneciente a la porcin radicular
del diente conlleva la aparicin de caries radiculares que debern ser restauradas
convenientemente.
Adems la recesin gingival conlleva repercusiones estticas, ya que si las
recesiones se producen en el sector anterior son visibles por el paciente y generan
preocupacin por su sonrisa poco esttica.

Numerosos estudios
principio es inocuo para los tejidos periodontales. Sin embargo, es evidente que el
mantenimiento de un estado de higiene correcto est entorpecido cuando un paciente es
portador de aparatologa ortodncica (Gkantidis y cols., 2012). Los movimientos que
sufre el diente durante el tratamiento s pueden modificar su posicin dentro de la
arcada, disminuyendo o aumentando las dimensiones de la enca queratinizada que
sostienen al diente y puede mejorar o empeorar su situacin gingival. Numerosos
estudios coinciden en la necesidad de cooperacin entre el periodoncista y el
ortodoncista durante el tratamiento de Ortodoncia para garantizar la salud periodontal
independientemente del tejido de soporte presente al inicio del tratamiento (Gkantidis y
cols., 2012; Peck, 2012; Vassalli, 2010).
Al analizar la bibliografa se aprecia controversia en cuanto a si la Ortodoncia
puede asociarse a la aparicin o progresin de recesiones gingivales. El tratamiento de
Ortodoncia basndose en la evidencia es la direccin adecuada que deberan tomar los
profesionales. Desafortunadamente, sin embargo, la mayora de las datos basados es la

comerciales guen el tipo de aparatologa que utilizamos e incluso la biomecnica de
nuestro tratamiento (Burrow, 2008). Las tcnicas no extraccionistas se promueven de
forma muy habitual entre muchos ortodoncistas. Estas tendencias estn influenciadas
por las ideas que preconizaba hace muchos aos Edward H. Angle y sus seguidores que
renunciaban a realizar extracciones durante el tratamiento ortodncico. Bsicamente,
para conseguir no realizar extracciones durante un tratamiento ortodncico para
solventar la falta de espacio, se recurre a una aparatologa que incluye la colocacin de
arcos de nickel titanio generosamente expandidos y mecanismos de distalizacin (Peck,
2012).
Reflexionando sobre en qu debera consistir la profesionalidad de un
ortodoncista, creemos que en nuestros das la manera de realizar el diagnsticos y el
plan de tratamiento dista mucho de lo que se consideraba correcto en la antigedad.
Hace aos la labor del ortodoncista se basaba en los planteamientos de las
maloclusiones que se le presentaba, detectando signos y sntomas para formular un
correcto diagnstico y as asesorar al paciente del mejor plan de tratamiento. Sin

Concluyen, por tanto, que una expansin con disyuncin moderada en adultos, de entre
1 y 5 mm, puede ser una alternativa vlida par evitar las extracciones y solventar el
apiamiento (Figura 16).
Otros estudios han tratado de medir mediante CBCT la altura de la cresta
alveolar en la superficie vestibular y lingual al inicio y al final de la expansin (Garib y
cols., 2006; Nguyen y cols., 2013) (Figura 17). Los resultados de Garib muestran una
reduccin significativa del BACL de los dientes de anclaje. Tambin se observ que la
RME caus una reduccin significativa del BBPT a nivel de molares y premolares e

Los sistemas de autoligado estn aumentado en popularidad entre los
ortodoncistas. El sistema Damon, concretamente, se anuncia como una tcnica que
supone avances para el paciente y para el ortodoncista, respecto a los brackets
convencionales e incluso se proclama superior a otros sistemas de autoligado (Wright y
cols.,2011; Bascifti y cols., 2014). En los aos 90, Dwight Damon desarroll una teora,
la cual propugnaba que la baja friccin y las fuerzas ligeras produca biolgicamente
resultados ms estables. Esta tcnica tambin defenda su mayor rapidez y la reduccin
de los tiempos de tratamiento, dada la baja friccin que se consigue supuestamente con
los brackets de autoligado. La filosofa Damon, contraria a las extracciones, resuelve los
apiamientos severos mediante arcos sobre expansionados muy ligeros y sostienen que
las fuerzas ligeras compatibles biolgicamente, no se enfrentan a la musculatura. En
otras palabras, los msculos periorales, actan a mode de lip bumper, minimizando la
vestibulizacin de los inicisivos inferiores. De acuerdo con este concepto, la forma de
arco se alinea tomando el camino de menor resistencia , que es la expansin posterior.
De esta manera, esta tcnica solventa los problemas de apiamiento dental mediante la
alineacin y nivelacin ortodncica, ajustndose a las dimensiones del arco que vienen
dadas, sin realizar extracciones. Implica un aumento en el permetro del arco, que se
logra a expensas del avance de los incisivos y de la expansin transversal (Bascifti y
cols., 2014). Damon tambin defiende que la anchura intercanina mandibular no cambia
significativamente con su sistema y los trazados cefalomtricos que encontramos en sus
artculos publicados muestran un movimiento mnimo de los incisivos hacia vestibular
(Vajaria y cols., 2011).
