AFFIDAVIT OF EXPLANATION

I, __________________ , of legal age, Filipino, and a resident of

_____________________________________, Davao del Sur, after having
been duly sworn to in accordance with law, do hereby depose and
state that:
1. I am currently employed as an active member of the
Philippine National Police (PNP);
2. I was recently officially assigned to the Presidential
Security Protection Group-Presidential Protection Unit
(PSPG-PPU) which is a unit of the Presidential Security
Group (PSG);
3. On ________________, I was selected as one of the police
officers to go to Manila on ___________ to attend a seminar
denominated as VIP Protection Orientation Seminar,
otherwise known as VIPPOS. The said seminar lasted for ten
(10) days;
4. I was able to return back to Davao City during the last week
of June 2016;
5. However, my stay in Manila coincided with the PNPs annual
Physical Fitness Test (PFT) which was scheduled on
_____________, when I was still in Manila attending the
aforementioned seminar;
6. By reason thereof, much as I want to, I was unable to attend
and take part of the above-mentioned annual physical
fitness test of the PNP held in Davao City;
7. I am executing this affidavit to attest to the truth of the
foregoing and to apprise the proper authorities of the reason
for my non-attendance during the physical fitness test and
for other legal purposes.
