Checklist of Requirements

PEZA Special Non-Immigrant 47(a)(2) visa


extension

Letter request from the Company for 47(a)(2) visa extension;

DOJ Application form duly accomplished and notarized

PEZA Application form duly accomplished and notarized

Photocopy of Companys PEZA Certificate of Registration

Original passport and photocopy of its relevant pages,


including the latest arrival and updated stay

Original Employment Contract or Assignment Contract

Copy of the renewed AEP Card or proof of filing of AEP


application duly Received by the Department of
Labor and Employment (DOLE) (to be secured by
ACCRALAW)

PEZA Global Services List of Expats

Latest PEZA EZMPR

Secretarys Certificate of Companys authorized signatory

Original and signed curriculum vitae by the foreign national


(including the current employer)

Latest Organizational Chart of the Company

Affidavit of Support and Guaranty in favor of the foreign


national

Copy of the Department of Justice (DOJ) First Indorsement


granting the 47(a)(2)(PEZA)

Original, hardcopy authorization letter of the company to


ACCRALAW to process PEZA visa extension

Email confirmation from the authorized signatories (draft to


be supplied)

If with dependents (legal spouse and/or unmarried children


21 years of age or below):

PEZA application form for the entry of spouse


and/or unmarried minor children duly accomplished and
notarized
Original passport and photocopy of its relevant
pages, including the latest arrival and updated stay

Photocopy of the marriage contract (with


translation in English if written in a foreign language other
than English)

Photocopy of the Birth Certificate/s of the


child/children (with translation in English if written in a
foreign language other than English)

Affidavit of support in favor of dependents signed


by the principal applicant

*REMINDERS*
(a) Please prepare 2 original copies of each document (except
for photocopies of marriage contract and birth certificate).
(b) The foregoing documents should be signed by the
Companys authorized signatories as enumerated in a
secretarys certificate (with the specimen signature/s) for
the visa application of its employees.
(c) The Company should be updated with its monthly
Economic Zone Monthly Performance Report.
(d) The Company should have submitted its PEZA Company
Satisfaction Survey.
(e) The applicant/s name/s found in all the supporting
documents must be the identical with the given name/s in
the passport.
