1. Write a brief Impression and Plan for this patient . The impression should describe the
location and possible causes of this patients problem. The plan should describe the next
steps (e.g. diagnostic work-up) you recommend for this patients care. You should turn-in
your brief write up (no more than 1/2 page) of the impression and plan to your instructor. If
needed, Blumenfeld section 1.2.10 (page 7) provides some useful guidance on how to
record an Impression (aka, assessment) and Plan.
Impression:
1. Ischemic cerebral infarction d/t occlusion of middle cerebral artery
2. Brochas/nonfluent aphasia
3. Atrial fibrillation
Plan:
1. Repeat CT
2. PTT/INR
3. Start long-term anticoagulant therapy
4. CXR
5. Speech therapy
