JOINT-AFFIDAVIT OF EXHUMATION
We, KIRBY G. PRUDENCIADO,of legal age, Filipino, single and
with residence and postal address at No. 37 Ipil St. Marikina Heights,
Marikina City, RACHELE P. APOSTOL, of legal age, Filipino, widow,
SHERWIN G. PRUDENCIADO,of legal age, Filipino, widow and
DANA G. PRUDENCIADO, of legal age, Filipino, single and with the
same residence and postal address at Phase 2A Block 3 Lot 7 San
Jose Del Monte Bulacan, after having been duly sworn to in
accordance with law, do hereby depose and state:
1. That we are biological children of the late FERNANDO J.
PRUDENCIADO, who died on October 5, 20O8 at 749
Tandang Sora Ave. Old Balara, Quezon and buried in
Bagbag Cemetery, Quezon City. Attached hereto is a copy
of his Certificate of Death with Registry No. 2008-13835
and made an integral part hereof;
2. That we are biological children of the late JOSEFA G.
PRUDENCIADO, who died on December 25, 2015 at
Kairos Maternity and General Hospital, Muzon, City of San
Jose Del Monte, Bulacan and buried in De La Paz Memorial
Chapel, Marikina City. Attached hereto is a copy of his
Certificate of Death with Registry No. 2015-2369 and
made an integral part hereof;
3. That we have decided that the skeletal remains of our a
forenamed parents be moved, transferred and exhumed
from BAGBAG CEMETERY, QUEZON CITY to LOYOLA
MEMORIAL PARK, MARIKINA CITY;
4. That my siblings who orally agreed, give their verbal
consent with regards to the exhumation, cremation and
re-burial of our beloved parents FERNANDO J.
PRUDENCIADO and JOSEFA G. PRUDENCIADO;
5. That we hereby undertake and declare to agree that in
behalf of our other relatives, We will be answerable and
liable and further declare that the Quezon City Health
Department will be free from any liability on said matter;
6. That we have executed this affidavit to attest to the truth
and veracity of all the foregoing facts for all legal intent
and legitimate purpose this may serve, especially for the

information of all the authorities concerned for the

exhumation of our beloved parents remains.
IN WITNESS WHEREOF, we have hereunto set our hands and
affixed our signatures this 28th day of December, 2015 in Quezon
City, Metro Manila, Philippines.
