A RESOLUTION REQUESTING ONE (1) AMBULANCE UNIT FROM THE

PHILIPPINE CHARITY SWEEPSTAKES OFFICE (PCSO) TO BE USED BY THE CITY

DISASTER RISK REDUCTION AND MANAGEMENT OFFICE (CDRRMO) AND THE
CITY HEALTH OFFICE

WHEREAS, Ormoc City was one of the hardest hit areas by Typhoon Yolanda last
November 8, 2013 and that the heavy wind and rain destroyed almost all the vehicles owned by
the city including the ambulance units;
WHEREAS, one of the basic and most important services that the government extends to
its people is health services;
WHEREAS, the PCSO is one such government entity that can greatly cater and extend
assistance to this kind of necessity by way of donating ambulance units to local government
units;
WHEREAS, the office of the City Mayor respectfully referred to this Honorable body
for a passage of a resolution requesting the Philippine Charity Sweepstakes Office One (1) Unit
of Ambulance to be used by the City Disaster Reduction and Management Office of this City in
responding to emergencies caused by natural and man-made calamities and the City Health
Office, also of this City as a service vehicle to ensure availability of transportation during
emergencies and other medical services;
WHEREAS, City Health Office of Ormoc has a total of three (3) ambulance units , two
(2) which are undergoing repairs while the remaining one (1) unit has been operating for about
nine (9) years already;
WHEREAS, the City Disaster Reduction and Management Office (CDRMO) only has
two (2) ambulance units both undergoing repairs and their service vehicle is now being
temporarily used as an ambulance;
WHEREAS, to this day, the City has only one (1) serviceable ambulance unit operating
within the area and catering to the sick people of one hundred ten (110) barangays;
WHEREAS, upon close and thorough review, this Sanggunian finds that the use of an
ambulance is necessary so that the local government unit (LGU) can deliver basic health services
and promote the general welfare of the people;

WHEREFORE, on motion of the SP Member Mario M. Rodriguez, Chairman,

Committee on Health and Sanitation, ____ seconded by the Honorable SP Member _________;

SWEEPSTAKES OFFICE (PCSO) TO BE USED BY THE CITY DISASTER RISK
REDUCTION AND MANAGEMENT OFFICE AND THE CITY HEALTH OFFICE

RESOLVED, FURTHER, to furnish copies of this resolution each to His Honor, the City
Mayor of Ormoc Richard I. Gomez, the City Administrator, the City Legal Officer, the OICCDRRMO, Mr. Ciriaco E. Tolibao, the City Health Officer, Dr. Nelita D. Navales, the General
Manager of PCSO, Mr. Jose Ferdinand M. Rojas II, and all other offices concerned.
