INTRODUCTION
Emergency procedures must be developed or updated for all potential incidents
including fire, explosion, toxic gas leaks, weather disturbances and civil unrest.
Procedures must include details on communications, fire fighting, medical, security,
evacuation, resumption of operations or others as required by the situation.
Emergency evacuation plans are developed by the TONSCO to include all work
areas, subcontractors, the office areas and any camps. Each subcontractor is
responsible for determining its own requirements and coordinating their plans and
efforts with the TONSCOs HSE and security requirements.
In action, the TONSCO efforts in an emergency must be integrated with
safety/security actions of the client. The site medical staff, where provided have
approved first aid training, which must be maintained current. CPR training is also
included.
The purpose of this procedure is to establish a plan of action for emergencies to
minimize the potential for injury, lose of life and/or property and to define the
responsibilities of managers, supervisors, employees, and subcontractors during
emergency situations.

All employees, subcontractors, vendors, and other personnel assigned to or visiting a
project site under our management.

4.
DEFINITION
1. The Emergency and evacuation, means to serious situation need quick action to
be anticipated all hazard occurred i.e. fire, explosions, leakages of toxic or
flammable gas or liquid, and natural calamities such as earthquakes, floods, etc.
2. Fire Accident, there is a large fire incident happened in the work location which
could increase potential fire hazards to some peoples concerned.
The small fires that can be extinguished immediately by person with using
portable fire extinguishers are not included to emergency situations.

PROCEDURE
6.1. EVACUATION
This is a generic example of an emergency evacuation procedure. Every location
must develop location specific procedures.

Identify the alarm signal for the type of emergency taking place.

Identify the direction of the wind and evacuate cross or upwind of the affected
area to a designated assembly area.
3.
If the designated assembly area is not accessible or safe due to wind direction,
proceed to an alternate assembly area.

All radio traffic should cease except for those authorized to broadcast
emergency information on the main radio channel.

Supervisors should begin head counts and report their status to the craft
supervisor.

All engine-driven equipment should be shut down and engines turned off.

10. Any permit voided due to an emergency must be properly reissued prior to
employees returning to their work activities.
11. No one should return to work unless an All Clear signal has been given.
12. Assembly Locations
a. Assembly points will be designated for all employees.
b. All assembly points will be well marked and highly visible.
c. Utilize the wind direction to determine an appropriate evacuation route and
assembly point.
d. Always select an assembly point that is upwind of the affected area.
13. Alarm System Signals
Each site must determine what alarm system and signal is to be used or
followed. An addendum should be added to this section.

Socks will be strategically located for employee ease in identification of wind
direction.

the alarm, communicating the nature and location of the alarm and making the
decision whether to conduct a complete evacuation and the assembly areas to
be use. He/she and client plant personnel are also responsible for the search
for personnel not accounted for within the unit areas, and for declaring that it
is safe to return to work once the responsible plant signals all clear.
2. The TONSCO General Site Manager will be the TONSCO Incident
Commander responsible for monitoring the situation on TONSCO radio
channel, ensuring a quick, effective evacuation, monitoring the head counts to
ensure all TONSCO personnel are accounted for. He/she, in coordination with
the Client Incident Commander, decides whether it is safe for TONSCO
personnel to return to work when the all clear is given.
3. The TONSCO HSE Manager is responsible for changing over to the client
radio channel to monitor the nature and location of the incident, the
routes/assembly areas to use to evacuate and to communicate with TONSCO
Incident Commander to ensure they are adequately informed of the current
status of the evacuation.
4. The TONSCO HSE Manager also monitors the head count reports to
personnel and helps conduct any assembly area searches for missing
personnel, as well as ensuring all safety requirements are met, such as no
smoking, stopping all work, turning off all engines and vehicles. Permits are
re-established before work is begun once the All Clear and return to work is
sounded.
5. The TONSCO Personnel Manager is responsible for tracking all personnel
onsite on a daily basis, for receiving headcount reports and tracking them
against the list of personnel on site. This list can be provided to the client
Shift Supervisor at the start of each shift, as required, to get a daily entry
permit issued by the client. He/she notifies the HSE Manager and TONSCO
Incident Commander of any personnel reported missing during the evacuation
head counts.

SOP - RADIOGRAPHIC EXPOSURE
Persons may accidentally be exposed to radiation emitting form an x-ray machine. This
may happen if the machine is unintentionally energized while persons are within the
danger zone. Accidentally be exposures may also occur if persons enter the barricaded
area.
As soon as the accident is discovered, the following actions should be taken.
i. Action by the Radiographer
-
Remove them from the works involving ionizing radiation until their doses have
been established.
Ask the competent authority concerned for assistance in assessing their doses. In
this country the most likely authority is either the Ministry of Labor or the Atomic
Energy Council (BATAN).
Send their film badges to an approved laboratory for urgent examination, and in all
cases: Make a full investigation of the incident, taking written statements from the
radiographer or any other persons involved, including details of where they were in
relation to the x - ray machine and for how long. If it appears that the radiographer
may have been overexposed, notify an employment medical advisor or appointed
doctor immediately so that the necessary medical examination may be carried out.

Since work should be conducted in a predetermined barrier and constant monitoring of the
dose is carried out using a survey mater, the presence of any unshielded source should
apparent immediately. In the event of this type of accident, the following action should be
taken:

SOP - SEVERE WEATHER
1. Lightning
-
When high wind creates a hazard to craftsmen or work being performed, i.e.,
instability in elevated areas, limited visibility due to dust or particles in the air,
unmanageable materials, etc., supervision will stop work activities, reassign work
and area, properly store and secure material which might blow away, injure or
damage, lower/tie down crane booms and obtain further instruction form TONSCO
Project Management.
When rain creates a hazard to craftsmen on work being performed, i.e., unstable
footing conditions due to slippery structural steel, muddy and flooded work
environments, unstable trenches or excavations, poor visibility due to rain or eye
protection, supervision will stop specific work due to hazard, re-assign work duties
and/or areas, and obtain further instructions from TONSCO Project Management.
3. Hurricanes/typhoons/Strom

Employees not at the project should listen to the radio. TONSCO Site Management
will have bulletined Broadcast informing employees as to when to return to work
or other instructions.
Post Hurricane/typhoon Clean Up and Project Construction Start-up
When the storm/hurricane/typhoon has passed, hazards may still exist because of
unstable structures or other hazardous conditions. Therefore, a safety and property
damage/loss assessment inspection shall be conducted on the project by
supervision and reported to TONSCO HSE Manager for planned clean-up prior to
start up or normal construction activities.

Do NOT touch it. Ask all persons to leave the area within the building.
Seal the area as best possible (e.g. block entrance).
Immediately go to another area and Call HSE/Security office.
Await further instruction and advice.
Give your name, location, number of people involved, details of medical
emergency. Await further instruction and advice.

Bomb Threat
Talk to the caller in a calm manner finding out as much information as you can.
Try to find out where the bomb is located and what time it is likely to explode. Keep
the caller talking as long as possible.
Attract a nearby person in your area and write a note alerting them of the situation.
If the threat involves this building have some person contact management to authorize
immediate evacuation of the building.
Notify Cost Centre Manager and await instructions.
DO NOT ACTIVATE THE ALARM SYSTEM.
Notify Security immediately.
Move to a safe area well away from the building.
Follow the direction of HSE and do not re-enter until directed.

SOP - EARTHQUAKE
1. If you are outside - move to an open area away from any buildings.
If you are inside a building - take shelter under tables, desks or in doorways.
Stay away from windows, shelving, and large free standing furniture.
2. After the earthquake, look for any injured persons in your area, if necessary.
3. If safe to do so, leave the building as soon as possible.

1. Hubungi Team Medis dan Safety, kemudian Supervisor anda secepatnya dan
jelaskan mengenai situasinya dan keadaan luka sikorban.
Contact Team Medic and Safety, then your Supervisor immediately and explain the
situation and any injuries.
2. Lakukan P3K / CPR bila anda mampu, jika tidak cari seseorang yang bisa
melakukan itu.
Provide first aid CPR if you are able, if not find someone who can.
3. Jangan memindahkan korban, kecuali untuk mencegah terjadinya luka
tambahan yang berakibat buruk bagi sikorban.
Do ot move victim except to prevent further injury.
4. Jaga sikorban sampai Team Medis / Safety tiba ditempat kecelakaan.
Care for injured until Team Medic / Safety arrive.
5. Jangan merubah atau memindahkan posisi kendaraan, sampai selesai
dilakukannya penyelidikan.

Penjelasan
Description
Contoh dari material yang berbahaya adalah ; gasolin, acid, kaustik, solvent, barang
yang mudah terbakar atau bahan yang mudah meledak dan bahan kimia yang
lainnya. Tumpahan dari material tersebut atau material lainnya yang berasal dari
tanki tetap/drum atau dari kendaraan pembawa BBM atau yang sejenisnya, dimana
tumpahan tersebut mencapai 50 liter atau 1/4 drum.
Examples of hazardous materials are gasoline, acids, caustics, solvents, flammable of
explosive substances, etc. spills of these materials or unknown materials from stationary
tanks / drums or from a mobile transport which exceeds 50 liters or 1/4 drum.

SOP - CHECK LIST FOR EMERGENCY PURPOSES
IN CASE OF MOTOR VEHICLE ACCIDENT :
1. Do not panic !
2. If the vehicles sustain a minor damage (fender bender, etc) and the vehicles are
blocking traffics, assist in marking the vehicle position on the road then vehicles to the
side of the roadway to allow normal flow of traffic.
3. Do not leave the scene.
4. Instruct someone to call the police and wait till the local authorities to arrive.
5. Provide information only to the police or local authorities.
