a. All info must be updated & paid registration receipt required for schedule pick up.
b. Computer stations will be available at schedule pick up.
c. 7th graders & students new to LexRich5 must have TDAP immunization before
starting school.
d. Acceptable documentation for address verification shown on page 4.
Annual Proof of Residence: If not already provided, Parent or Legal Guardian must
provide THREE (3) Proofs of Residency Annually for student(s) to attend LexRich5.
All documentation presented must show the CURRENT MONTH'S DATE and the PROPERTY OWNER'S NAME (Affidavits Required) or the PARENT/GUARDIAN'S NAME.
Acceptable documentation includes: (One Required)
1. Property Tax Bill for (permanent residence) 2. Rental Lease (complete copy,
listing all occupants)
3. Real Estate Closing Documents (where applicable)
Additional documents include: (Two Required)
1. Vehicle Tax Registration 2. Voter Registration 3. Electric Bill

We are excited to announce the district has implemented an
on-line student registration system. Each school year, all
returning students to District Five must re-register to attend
their school. This is done to ensure all contact information
and other pertinent information is up to date.
Parents are asked to log into Registration Gateway to verify/
update their student(s) information for the 2016-2017 school year. You will be able to
verify and update contacts, phone numbers, medical conditions, review annual school
and technology agreements, and your students privacy selection. You will also be able
to complete the Media Consent forms, District Acceptable Use Agreements, Health
Card Information, and other important information.

You should have received an email providing your PowerSchool identification and password. If you have not received the email or need information or assistance, please call
your school's registrar. We hope that this new system will make the re-registration

Its time to elect our School Improvement Council (SIC) parent representatives. Irmo Middle School will continue
to excel with your assistance and involvement. The SIC is comprised of six elected parents, two teachers, two
staff members, three elected students from each grade (6th, 7th and 8th grade), three to seven appointed representatives from the school and community, and five ex-officio members. The SIC at Irmo Middle serves as a liaison between the school and community by bringing together parents, educators, students, and community
stakeholders to collaborate in the continuous improvement process for our school. Meetings will be held at the
school on the third Tuesday of each month at 5:30 pm.
We will begin accepting nominations August 1, 2016 through August 15, 2016. Nominations are accepted for
any parent/guardian of an Irmo Middle School student who would be willing to serve a two-year term for rising
6th and 7th graders and a one-year term for parents of rising 8th graders for the 2016-2017 school year. Any parent interested in serving or those who wish to submit another parents name should do so with their permission
using the nomination form below. Nominations may be mailed to the address below, hand delivered to IMS in a
sealed envelope or emailed to Robert Jackson at SJackso@lexrich5.org.
The election to vote for the SIC members will be held Thursday, August 25, 2016 at IMS from 7:30 am to 6 pm.
The list of nominees will be posted on our website prior to the election. Please complete the form below if you
are interested in running for the 2016-2017 SIC and returning by August 15th.
