Basic Function
Under general supervision, manages the nursing care service through
mentoring and monitoring work of nursing staff and students; performs
regular staff functions when necessary.
Authority
Directly supervises the Nursing Attendant II.
Reporting Arrangements
Reports directly to the Nurse III
Duties and Responsibility
Endorses and receives endorsement to and from the incoming and
preceding shift paying particular attention to:
1. Patients Census
2. Serious patients
3. Special treatment to be done or to be carried out
4. Unusual occurrence
Plans and direct s nursing activities in the unit with regard to patient care
and unit management.
Validates and carries out doctors order.
Proper documentation of nursing care rendered.
Ensures that drugs, supplies and equipment are used economically and
carefully.
Coordinates activities and relates with other departments for the welfare
of the patients.
Interprets and implements the policies and regulations of the hospital to
staff, patients and families.
Accomplishes accurate and informative legible OR and ward records such
as:
1. Patients Charts
2. Medicine, injection, IVF, diet and treatment list
3. Nurses Logbook
4. 24-hours report (10-6 shift)
5. Monthly inventories
Coordinates and participates in the overall educational and research
program of the hospital.
Performs as the duties and responsibilities of an OR Circulating Nurse,
Scrub Nurse and PACU nurse. Refer to the FM-NS-OR-021 (OR Nurses

e. Basic Function
Under general supervision, supervises the Nursing care service unit
through mentoring and monitoring work of nursing staff in the hospital;
performs regular staff functions; and assist in developing applicable
policies, rules, and regulations for the hospital.

Directly supervises Nurse II, Midwife II, Midwife I and Nursing Attendant II
g. Reporting Arrangements
Reports directly to the Nurse IV
preceding shift.
Proper documentation of nursing care rendered.
Checks the Attendance of the nursing personnel and makes the necessary
in care of absence of personnel are in their complete prescribed uniform
while on duty.
Ensures that all nursing personnel are in their complete prescribed
Guides the staff nurses, plan the OR activities and the assignment of
activities.
Assume leadership roles, create and maintain safe working conditions
Operating Room.
Supervises the staff nurses and nursing attendants in making monthly
inventories of linen, articles and equipment used in their respective units.
Makes requisitions and sees that drugs, supplies and equipment are used
economically and carefully.
Reviews and check 24 hour statistical reports when in night duty for
submission.
Ensure strict observance of professional ethical standards and reports any
misbehavior or misdemeanor of nursing personnel during his/her tour of
duty.
Submit collected incident reports to the Nurse IV for preliminary
investigation and appropriate action.
Evaluates performance of nursing personnel under her supervision every
two months and submits to the Nurse IV.
Attends to repair and proper maintenance of facilities, equipment and
supplies.
Conducts orientation of the new nursing personnel.
Participates in nursing studies to keep pace with scientific advances and in
support with on-going research in the hospital including adaptability to
nursing program.
Attends meeting and conferences/or through a delegate to gain for
Appraisal Checklist).
Performs other related functions as delegated.

Under general supervision, performs safe transport and positioning of
patient, maintains cleanliness of the area, assists in the inventory of the
medical equipment and supplies, reprocessing of surgical instruments and
accomplishes tasks as delegated by the Nurse.

h. Duties and Responsibilities
Do proper endorsement from the incoming shifts.
Maintains a therapeutic and safe environment for OR patients.
Assist personal and comfort needs of the patients.
Assists patients posture, activity and rest.
Changes wet clothes and bedding immediately and as scheduled.
Reprocessing of medical supplies and instruments.
Follow up of job orders status in the respective departments.
Monitors the functionality and inventory of the medical equipment
Maintains the Environmental Sanitation of the Operating Room.
Transport patient from the ward to OR and from the Operating room to
PACU, to ward and other units as prescribed.
Bring specimen to the laboratory.
Replace empty oxygen tanks at the OR and PACU.
Obtains drugs and medicines from the pharmacy with proper inspection
for expiration date and sterile supplies from the Central Supply Room for
OR use.
Ensures that proper Health Waste Management is implemented.
Transports the soiled linens to the Linen Section for laundry and obtains
the clean linens to maintain its availability in the OR.
